<br />? <br />"'U L,3 ?.'II!-ISt3te <br />hemity <br />Federal Work-Study, Off-Campus Contract <br />EXHIBIT A <br />o <br />Cleveland-Marshall College of Law <br />Financiat Aid Office <br />1801 Euclid Avenue <br />Cieveland, Ohio 44115 <br />Telephone: (216) 687-2317 <br />FAX: (216) 687-6881 <br />This agreement is entered into between Cleveland State University, hereinafter known as "University" and <br />City of North Olmsted, hereinafter known as "Agency," a local pubJic agency or private non-profit organization, for the <br />purpose of providing work to students eligible to participate in the Federal Work-Study Program. <br />Whereas, the Agency desires to utilize the services of one or more students participating in such Program. <br />It is hereby agreed as fotlows: <br />The Agency shall provide to the University for review and approvai a written job description of each position for which <br />services may be sought. The agency shall furnish adequate, qualified supervision of students placed with the Agency. Students <br />shall be assigned only to jobs approved by the University. <br />2. The Agency has the right to contro] and direct the services of students, not only as to the result to be accomplished, but also as <br />to the means by which the result is to be accomplished. Tlae Agency's employment policies shall apply to the extent such <br />policies are consistent with this Agreement. <br />Compensation of students for work approval and performed under this Agreement will be disbursed to each student by the <br />Agency. The Agency shall provide to the University a detailed Record of Hours Worked (attached) and a Billing Statement <br />(attached). <br />The Agency shall be the employer of record whicii shall include but not be limited to the responsibility for deductions for appropriate <br />taxes, Social Security, and for the purposes of Workers' Compensation statutes. <br />Based upon these time reports, the Agency will subsequently submit a Bilting Statement to the University. Accompanying documenis <br />wilt be the Record of Hours Worked inciuding the name of each student, each studenYs hourly wage rate, the number of hours each <br />student worked for the applicable period, each student's gross pay, Fiscal Year To Date Gross pay and the total sueh earnings during <br />each such pay period. The University agrees to pay the Agency, within thirty (30) days of receipt of each billing from the Agency, an <br />amount equal to seventy-five percent (75%) of the gross earnings. The University is responsible for reviewing prospective work assignments, determining whether individual students meet the <br />eligibility requirement under Federal Work-Study Program, assigning students to work for the Agency, and determining that <br />the students do, in fact, perforrrt their work. Such determination is fhrough site visits. <br />5. The number of students to be placed with the Agency and the hourly rate(s) of pay shall be as mutually agreed between the <br />Agency and the University. <br />6. No student shall work more than twenty (20) hours in any week during which a class or classes in which that student is <br />enrolled is/are in session, nor more than forty (40) hours during break periods or Summer Quarter. Any student in violation of <br />these requirements is subject to loss of work-study e(igibility under federal regulations. The law school strongly encourages <br />fuil-time students NOT to work at all during the first year of law study. <br />7. The Agency shall issue all paychecks to the student, but shall have sole responsibility for the payment of all time worked over <br />and above the work award amount shall not be reimbursed by the University. The Agency will provide a W-Z statement to the <br />student for all hours worked. The W-2 must reflect total compensation and taxes withheld. <br />data\cws\convact. do c <br />
