19.4 In conjunctian with participation within a medical plan, the full-time Employee may subscribe to and participate in tlze following optional health benefit coverage(s): l. Dental 2. Vision - Option Plan #I (vision plan which was in effect August l, 1994) Vision - Option Plan 92 (vision plan similar to that offered through Unian Eye Care) 3. Dental and Vision (Option Plan 41 or 42) Full-time Employees pazticipating and subscribing to the optianal health benefit Iisted above within tnis Section shall contribute an equa.l amount to that which is listed in Section 19.2(A), (B) and (C) of this Eirticle. Employees sha11 be required to participate uithin the aptiona.i benefit coverage listed within this Sectian. The Emplayee contribution shall be withheld as a payroll deduction not Iater than the first pay periad of each month. Dentat: AFSCN!lE Care Plan I3ental II(inctuding orthodontics) The City shall pay the entire cost of said AFSCME Care Plan Dental II In addition ta said dental coverage, in conjunction with participation within a medical plan, the full-time Employee may subscribe to and participate in the following health benefit coverage: 1. Vision - Option Plan #1 (vision plan which was in effect August l, 1994) 2. Vision - Option Plan #2 (vision plan similar to that offered through Unian Eye Care) Full-time Employees participating and subscribing to the optional health benefit listed above within this Section shall contribute an equal amount to that which is listed in Section 19.2 of this Artzcle.
