APPENDIX B <br />CITY OF NORTH QLMSTED <br />NEW BRANCH LIBRARY <br />Scope of Services <br />A. Intent of the Testing and Inspection Services: The City has prepared this RFP <br />with the intention of retaining the services of a Material Testing firm to: <br />1. Provide various material tests and inspection for the construction of the New <br />Branch Library as directed by the City of North Olmsted. <br />B. Basic Scope of the Testing and Inspection Services: In providing the Scope of <br />Services, the testing firm will place special emphasis on: <br />1. Provide various material tests as specifically stated in the following Section C. <br />2. All work shall be coordinated with the City's Construction Manager, H.R. Gray <br />& Associates, Inc., 12 E. Exchange St., 4d' Fl., Akron, Ohio 44308, 330-379- <br />3200, 330-379-3224 Fax. <br />3. Unit prices shall include all incidental costs including clerical work and <br />preparation of reports and results of the specified tests. <br />C. Specific Scope of the (Major) Phase Services: In providing the Scope of Services, <br />the testing firm shall include the following specific duties: <br />1. Asphalt Pavement Density Tests <br />2. Slump Test, Air Entertainment Test, Concrete Cylinder Preparation (includes <br />reports). <br />3. Cure and break concrete cylinders (includes reports) <br />4. Evaluation of precast Manufacture's QC. methods as directed by CM. <br />5. Mortar Tests per ASTM C780 <br />6. Grout Tests per ASTM C 1019 <br />7. Brick Tests per ASTM C67 <br />8. CMLT Tests per ASTM C140 <br />9. Prism Tests per ASTM C1314 <br />10. High strength bolted connection inspection <br />11. Weld Inspection <br />12. Unspecified Struct. Steel Testing as directed by CM <br />13. Unspecified Paint Testing as directed by CM <br />
