<br />CITY OF NORTH OLMSTED ORDINANCE NO. 68-#3 PAGE 2 <br />1: Personal illness, injury, or exposure to contagious disease <br />where quarantined by the Board of Health. <br />2: Illness in immediate family with a maximum of 15 work <br />days per year. The term " immediate family " in case of <br />illnflss shall be interpreted as meaning parents, regardless <br />of place of residence, and relatives living within the house- <br />hold of the employee. <br />3: Each death in the immediate family, a maximum of five <br />work days, including travel time. The term " immediate <br />family " in: the case of death shall be interpreted as meaning <br />parents, parents of spouse, spouse, and children of the <br />employee, regardless of place of residence, as well as other <br />relatives living in the?;employees's home. <br />4: Each death of brother or sister of employee or spouse, <br />five work days. Necessary travel time would be included <br />in that period. <br />C: Proof of Absence <br />The Department Head shall require that the employee <br />furnish a satisfactory affidavit to the effect that the <br />absence was caused by illness, or due to any of the <br />foregoing causes. <br />D: Return to Full Time Duty <br />Employees who have been absent for three days or more, <br />due to illness, shall be required to present a statement <br />from a physician validating that they are physically able <br />to resume their duties on a full-time basis. <br />When an employee has been absent for less than three <br />days, the department head, at his discretion, may re- <br />quest a statement from the employee or his physician <br />before the employee may return to work. <br />At the discretion of the Department Head, Director or <br />Mayor, the City Physician may be requested to call upon <br />any City employee. <br />In the event any employee has been off duty for a serious <br />ailment such as a heart attack, or any employee has suffered <br />loss of limb, sight, physical or mental disability, and if any <br />such employee is unable to perform the duties he or she per- <br />formed prior to his or her disability, the Director or Mayor <br />may deem it necessary to have the employee approved by the <br />City Physician before said employee goes back to work. <br />Decision as to whether the employee is capable of fulfilling <br />his or her duties shall be at the discretion of the Director or <br />the Mayor. If not, it will be necessary to wait for approval <br />by the City Physician.
