~.. <br />~," <br />a, <br /> brother of spouse; sister of spouse; grandparent, <br /> grandchild; as well as relatives living in the <br /> employee's home. <br />(e) Employees may use up to fifteen (15) work days <br /> per year of earned and accumulated sick leave if <br /> necessitated by illness in the employee's immediate <br /> family. For purposes of this subsection, "immediate <br /> family" shall be interpreted to be spouse; children; <br /> parents of the employee, regardless of the placeyef <br /> residence; and other relatives living within the " <br /> household of the employee. _ <br />(f) When an employee with five (5) or more years of <br /> service retires, having received P.E. R.S. pension <br /> and having accumulated sick leave time, the retiring <br /> employee may convert one-quarter (4) of the accumulated <br /> sick leave time to pay on the basis of his rate of pay <br /> at the time of retirement, but the maximum amount paid <br /> shall not exceed two hundred and forty (240) hours pay. <br />(g) An employee should make an effort to schedule medical <br /> and dental appointments for himself and his family <br /> members during non-work hours. <br />(h) Sick leave taken the day prior to or the day after a <br /> scheduled vacation or holiday will require a medical <br /> excuse prior to approval for payment. <br />35. General Leave Provisions.' <br />(a) All leaves of absence when known in advance (and <br />(and any extensions thereof.) must be applied for and <br />.granted in writing on forms provided by the City <br />(copy to the employee). An employee and the Union <br />will be notified within three (3) working days from <br />the date the application was made of approval or <br />.denial of the request leave. <br />-14- <br />
