- 2 - <br />(1) Fees for filing grievances and appeals; <br />(2) Fees for reviewing accidental discharge procedures and <br />construction required by Section 917.03; <br />(3) Fees for monitoring, inspections sample analysis and <br />surveillance procedures for scheduled and demand sampling <br />as warranted in the City's industrial pretreatment program; <br />(4) Fees for the City's administration costs associated with <br />industries required to maintain a scheduled sampling and <br />analysis program, as warranted in the City's industrial <br />pretreatment program. Fees shall be the total cost per year <br />for industry incurred by the Service Director, City <br />Engineer, Wastewater Treatment Plant Superintendent, <br />Pretreatment Coordinator plus miscellaneous costs for office <br />equipment, supplies, paper, copying and transportation. <br />(5) The funds received from the collection of fees for the <br />pretreatment program shall be deposited as received with the <br />Director of Finance who shall keep the same in the Sewer <br />Revenue Fund. <br />(c) Industrial Wastewater Discharge Permit <br />(1) Permit Required It shall be unlawful to discharge Industrial <br />Wastes to the POTW without first obtaining an Industrial Wastewater <br />Discharge Permit and complying with requirements stipulated in the <br />Industrial Wastewater Discharge Permit issued by the City. <br />(2) Application All Industrial Users, and other Users as may be <br />required by the Service Director, shall submit an application for <br />an Industrial Wastewater Discharge Permit to the Service Director <br />at least 90 days prior to connecting to or discharging to the POTW. <br />All existing Significant Industrial Users connected to or <br />discharging to the POTW, and which have not previously applied for <br />an Industrial Wastewater Discharge Permit, shall make application <br />to the Service Director for a Discharge Permit within 90 days of <br />the effective date of this Ordinance. Each application shall <br />include: <br />a. All information required by Sec. 917.05. <br />b. A list of any environmental control permits held by the <br />facility. <br />c. A description of operations, including the nature, rate <br />of production and Standard Industrial Classification (SIC) <br />of the operation(s). This description shall include a <br />schematic process diagram which indicates the point(s) of <br />discharge to the POTW. <br />
