SECTION 6: Section 917.05 shall be amended to add a new Section 917.05 <br />e to read as follows <br />"(e) All reports required by this chapter of the Codified Ordinances <br />must ~ signed by an authorized representative as defined in Section <br />911.03 (5) and all reports mist include the following certification <br />statement: <br />I certify under penalty of law that this <br />doctunent and all attachments were prepared <br />under my direction or supervision in <br />accordance with a system designed to assure <br />that qualified personnel properly gather and <br />evaluate the information submitted. Based on <br />my inquiry of the person or persons who <br />manage the system or those persons directly <br />responsible for gathering the information, the <br />information submitted is to the best of my <br />knowledge and belief, true accurate, and complete. <br />I am aware that there are significant <br />penalties for submitting false information,including <br />the possibility of fine and imprisonment for knowing <br />violations." <br />SECTION 7: Section 917.06 (f) of the Codified Ordinances of the City of <br />North Olmsted, Ohio which now reads as follows: <br />"(f) Enforcement Actions• Annual Publication. A list of all <br />significant dischargers which were the subject of enforcement <br />proceedings pursuant to the Codified Ordinances during the twelve <br />previous months shall be annually published by the City in the largest <br />daily newspaper serving the Municipality. The public notice will <br />summarize the enforcement actions taken against the dischargers during <br />the same twelve months whose violations remain uncorrected forty-five or <br />more days after modification of noncompliance or which have exhibited a <br />pattern of noncompliance over that twelve month period, or which involve <br />failure to accurately report noncompliance." <br />BE AND THE SAME IS HEREBY AMEi~TDID, AND AS AMENDED SHAT,T, READ AS FOIII,OLJS <br />"(f) At least annually, the Service Director shall publish a list of all <br />industrial users which at any time during the previous twelve months <br />were in significant noncompliance with applicable pretreatment <br />requirements. For the purpose of this provision, an industrial user is <br />in significant noncompliance if its violations meet one or more of the <br />following criteria: <br />(1) Chronic violations of wastewater <br />here as those in which sixty-six <br />of the measurements taken during <br />any magnitude) the daily maximum <br />for the same pollutant parameter; <br />discharge limits, defined <br />percent or more of all <br />a six-month period exceed (by <br />limit or the average limit <br />-7- <br />
