_ _ w w,....M.?,..??,.?..?...,_ . <br />. . .?, <br /> absent for less than a full day); <br />(c) If an employee is absent on sick leave for more than <br /> three (3) consecutive work days, the City shall require <br /> the employee to present medical evidence that the <br /> employee was under a doctor's care and is able to <br /> return to work. The City will review the attendance <br /> records of employees periodically and at least once <br /> in each three (3) month period. If it believes that <br /> an employee is developing an attendance problem, it <br /> will notify the employee in writing that medical <br /> evidence will be required for absences of three (3) <br /> days or less. When an employee's record becomes <br /> satisfactory, the requirement for such medical <br /> evidence for short term absences will be removed, and <br /> the employee will be notified in writing. ? <br />(d) Employees may use up to fifteen (15) work days per year <br /> of earned and accuYnulated sick leave if necessitated by <br /> illness in the employee's immediate family. For <br /> purposes of this subsection, "immediate family" shall <br /> be interpreted to be spouse; children; parents of the <br /> employee, regardless of the place of residence; and <br /> other relatives living within the household of the <br /> employee. <br /> <br />} (e) When an employee with five (5) or more years of service <br /> retires, having received P.E.R.S. pension, dies or <br /> takes disability retirement and has accumulated sick <br /> leave time, the retiring employee or his estate may <br /> convert one-half (1/2) of the accumulated sick leave <br /> time to pay on the basis of his rate of pay at the time <br /> of retirement, but the maximum amount paid shall not <br /> exceed the following: <br /> Five hundred (500) hours in period <br />, 7/19/92 - 6/30/93 <br /> Six hundred (600) hours in period <br /> 7/1/93 - 6/30/94 <br /> Seven hundred (700) hours in period <br /> after 7/1/94 <br />(f) An employee should make an effort to schedule medical <br /> and dental appointments for himself and his family <br /> members during non-work hours. <br />(g) Sick leave taken the day prior to or the day after a <br /> scheduled vacation or holiday will require a medical <br /> <br />-•-., x <br />excuse prior to approval for payment. <br />?. ? .: <br />Section 4.
