CITY OF NORTH OLMSTED <br />RESOLUTION N0.2011 - 88 <br />By: Mayor Kennedy <br />A RESOLUTION AUTHORIZING THE MAYOR TO ENTER INTO A <br />CONTRACT WITH Ci~S PHARMACY, INC. FOR THE ADMINISTRATION OF <br />INFLUENZA i~ACCINATIONS AT THE NORTH OLMSTED SENIOR CENTER. <br />WHEREAS, it is the desire of the Mayor to enter into a contract on behalf of the City of <br />North Olmsted with CVS Pharmacy Inc. for the administration of influenza vaccines at the North <br />Olmsted Senior Center; and <br />WHEREAS, the CVS Vaccination program at the Senior Center is proposed for October lOt~' <br />through 14`", 2011, at no cost to the City of North Olmsted and for the benefit of North Olmsted <br />residents; and <br />WHEREAS, it is the desire of this Council to authorize the Mayor to enter into said contract <br />in order to obtain said services for the benefit of the senior population. <br />NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE CITY OF <br />NORTH OLMSTED, COUNTY OF CUYAHOGA, AND STATE OF OHIO: <br />SECTION 1: That the Mayor be and he is hereby authorized to enter into a contract with <br />CVS Pharmacy Inc. for the administration of influenza vaccines at the North Olmsted Senior Center. <br />Said contract shall be substantially in the form of the "Influenza Vaccination Agreement" marked as <br />Exhibit "A" and attached hereto, with only such changes therein as are not substantially adverse to the <br />City and which are approved by the Director of Law. <br />SECTION 2: That this Resolution shall take effect and be in force from and after the earliest <br />period provided by law. <br />PASSED: ` p~" ~ <br />ATT <br />,~ <br />~~ <br />MARIE GALLO <br />Clerk of Council <br />First Reading: _ <br />Second Reading.• <br />Third Readin <br />Committee.~_~-~1~ <br />y ~ ~ <br />_ :~ r=~ <br />DUANE K.
