CITY OF NORTH OLMSTED <br />RESOLUTION N0.2011 - 112 <br />By: Mayor Kennedy <br />A RESOL UTIONA UTHORIZING THE MAYOR OF THE CITY OF NORTH <br />OLMSTED TO ENTER INTO ATWO-YEAR CONTRACT WITH UNION EYE <br />CARE CENTER, INC., TO PROVIDE EYE CAREAND VISIONSERVICES TO <br />CITY EMPLOYEES IN CONNECTION WITH THE CITY'S SELF-INSURED <br />HEALTH CARE PLAN, AND DECLARING AN EMERGENCY. <br />WHEREAS, it is the desire of this Council, based upon the recommendation of the <br />Director of Human Resources, to authorize the Mayor to enter into atwo-year contract with <br />Union Eye Care Center, Inc., to provide eye care and vision services to City employees in <br />connection with the City's Self-Insured Health Care Plan, as most recently approved by the <br />Employee Health Care Committee; and <br />NOW THEREFORE, BE IT RESOL VED BY THE COUNCIL OF THE CITY OF <br />NORTH OLMSTED, CUYAHOGA COUNTY, AND STATE OF OHIO: <br />SECTION l: That the Mayor be and he is hereby authorized, subject to approval by the <br />Board of Control, to enter into a contract with Union Eye Care Center, Inc., to provide eye care <br />and vision services to City employees in connection with the City's self-insured health care plan. <br />The contract shall be substantially in the form of the "City of North Olmsted Vision Care <br />Agreement," attached to this Resolution and marked as "Exhibit A." <br />SECTION 2: That the contract shall be for atwo-year term effective from January 1, <br />2012, through December 31, 2014. <br />SECTION 3: That this Resolution is hereby declared to be an emergency measure <br />necessary for the preservation of the public health, safety, and welfare, and for the further reason <br />it is necessary that this legislation immediately go into effect upon passage due to the fact that <br />the current coverage for employees will expire on December 31, 2011 - a date before the <br />effective date of this resolution if not passed under emergency; and further, provided it receives <br />the affirmative vote of two-thirds of all members of the Council, it shall take effect and be in <br />force immediately upon its passage and approval by the Mayor. <br />PASSED: /r~~-7~~CIl~lL,~~..
