5.0 Has any officer or partner of your organization ever been an officer or partner of another <br />organization that had any construction contract terminated by the owner; terminated wozk on a <br />project prior to its completion for any reason; had any surety which issued a performance bond <br />complete the work in its own name or financed such completion; or had any surety expend any <br />monies in connection with a contract for which they furnished a bond? If the answer to any <br />portion of this question is "yes", please furnish details of all such occurrences including name of <br />owner, architect or engineer, and surety, and name and date of project. <br />6.0 List name pf project, owner, architect or engineer, contract amount, percent complete and <br />scheduled completion of the major construction projects your organization has in process on this <br />date. <br />7.0 List name of project, owner, architect or engineer, contract amount, date of completion and <br />percent of work with own forces of the major projects of the same general nature as this project <br />which your organization has completed in the past five years. <br />8.0 List name, address and telephone number of a reference for each project listed under Items 6.0 <br />and 7.0, above. <br />9.0 List name and construction experience of the principal individuals of your organization. <br />10.0 List the states and categories of construction in which your organization is legally qualified to do <br />business. <br />11.0 List name, address and telephone number of an iiidividuai who represents each of the foliowing <br />and whom OWNER may contact for a financial reference: <br />11.1 A surety: <br />11.2 A bank: <br />113 A major material supplier: <br />ADDITIONAL CRITERIA FOR DETERMINING THE RESPONSIBILITY OF THE BIDDER: <br />In addition to the primary "responsibility qualifications "information requested above, each Bidder shall <br />furnish the following information regarding the following items. <br />a. Description of its experience with projects of comparable size, complexity, and cost within <br />recent years, demonstrating the contractor's ability and capability to perform a substantial <br />portion of the project with its own forces; <br />b. Documentation from previous projects regarding timeliness of performance, quality of work, <br />extension requests, fines and penalties imposed and payment thereof, liens filed, history of <br />claims for extra work, contract defaults, together with explanations of same; <br />c. Identification and description of any projects within the previous five years that the Bidder <br />was determined by a public entity not to be a responsible contractor, the reasons given by the <br />public entity, together with an explanation thereof; <br />An adequate demonstration of financial responsibility, which may include in the Service <br />Director's discretion, a certified financial statement prepared by a certified public accountant, <br />to assure that the Bidder possesses adequate resources and availability of credit and the <br />means and ability to procure insurance and bonds required for the project; <br />BQS-2 <br />
