5.0 Has any officer or partner of your organization ever been an officer or partner of another organization that had any construction contract terminated by the owner; terminated work on a project prior to its completion for any reason; had any surety which issued a performance bond complete the work in its own name or financed such completion; or had any surety expend any monies in connection with a contract for which they furnished a bond? If the answer to any portion of this question is "yes", please furnish details of all such occurrences. including name of owner, architect or engineer, and surety, and name and date of project. Description of its experience with projects of comparable size, complexity, and cost within recent years, demonstrating the contractor's ability and capability to perform a substantial portion of the project with its own forces; b. Documentation from previous projects regarding timeliness of performance, quality of work, extension requests, fines and penalties imposed and payment thereof, liens filed, history of claims for extra work, contract defaults, together with explanations of same; c. Identification and description of any projects within the previous five years that the Bidder was determined by a public entity not to be a responsible contractor, the reasons given by the public entity, together with an explanation thereof; d.
