' NextPage LivePublish <br />Page 2 of 4 <br />(c) The minimum deposit for pavement borings or openings referred to in subsection (a) hereof <br />shall be as follows: <br /> <br />(1) Brookpark Road, Columbia Road (S.R. 252 south of <br />Butternut Ridge Road), Great Northern Boulevard and Lorain <br />Road: $3,000 <br />(2) Al Moen Drive, Brookpark Road Extension, Country Club <br />Boulevard, Dover Center Road, Industrial Parkway and Sparky <br />Lane: $2,500 <br />(3) Barton Road, Bradley Road, Butternut Ridge Road, <br />Canterbury Road, Cedar Point Road, Clague Road, Columbia <br />Road (except S.R. 252 south of Butternut Ridge Road), Fitch <br />Road, Gessner Road, Kennedy Ridge Road (between Victoria <br />Lane and Butternut Ridge Road), McKenzie Road, Mastick <br />Road, Mill Road, Porter Road, Stearns Road, Victoria Lane and <br />Walter Road: <br />$2,000 <br />(4) All other streets with concrete pavement and curbing: $1,500 <br />(5) All other streets not included in items (1) through (4): $750.00 <br />(Ord. 2007-155. Passed 12-18-07.) <br />901.02 ISSUANCE REQUIREMENTS. <br />No such permit shall be issued until: <br />(a) The applicant shall have made the deposit referred to in Section 901.01 or 901.07. Such permit <br />shall state the exact time the opening or excavation is to be made, the probable length of time for doing <br />the work and the making of the repair, the character of the pavement, including its base, and the <br />character of the work necessary for the satisfactory repairing or relaying of the roadway. <br />(b) The applicant shall have submitted a policy or policies of liability or indemnity insurance <br />issued by companies licensed to do business in the State, recognizing the City as an insured, in a total <br />amount of not less than five hundred thousand dollars/one million dollars ($500,0001$1,000,000) for any <br />personal injury or injuries incurred and two hundred thousand dollars ($200,000) for any property <br />damage caused as a result or consequence of the making or existence of such opening or excavation, or <br />the condition of the road, street, sidewalk, tree lawn or other public property in which the same is or has <br />been made. <br />(Ord. 2007-155. Passed 12-18-07.) <br />901.03 NOTIFICATION OF POLICE DIVISION. <br />Upon the issuance of a permit as set forth in Section 901.02, it shall be the duty of the City Engineer <br />to advise the Police Division of the location, time and duration of the street opening, in order that <br />preparation can be made by the Police Division for traffic control in the area of the street opening. <br />(Ord. 96-26.
