TERM OF AGREEMENT - This Program will be in force from <br />January 1, 2012 to December 31, 2013. The City of North Olmsted <br />retains the option to renew the program for an additional two-year <br />term by providing a thirty-day written notice to Union Eye Care <br />Center, Inc. Rates during this option terrn are to be negotiated but <br />are not to exceed 4%. <br />Union Eye Care is offering a two-year renewal term at the current <br />rates aad benefit levels through 12/31/2015. Therefore the monthly <br />rates will remain unchanged, as follows. <br />Single Plan $3.38 <br />Family Plan $9.43 <br />Please let me know if the City would be interested in this renewal <br />extension.
