?,. <br />? <br />CITY OF NORTH OLMSTED <br />RESOLUTION NO. 2006 - 201 <br />BY: Mayor O'Grady <br />? <br />? <br />A RESOL UTION A UTHORIZING THE MA YOR OF THE CITY OF <br />NORTH OLMSTED TO ENTER INTO A TWO-YEAR CONTRACT <br />WITH UNION EYE CARE CENTER, INC., TO PROVIDE EYE CARE <br />AND VISION SERVICES TO CITY EMPLOYEES IN CONNECTION <br />WITH THE CITY'S SELF-INSURED HEALTH CARE PLAN, AND <br />DECLARING AN EMERGENCY. <br />WHEREAS, it is the desire of this Council, based upon the recommendation of <br />the Director of Human Resources, to authorize the Mayor to enter into a two-year <br />contract with Union Eye Care Center, Inc., to provide eye care and vision services to City <br />employees in connection with the City's Self-Insured Health Care Plan, as most recently <br />approved by the Employee Health Care Committee; and <br />NOW THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE CITY <br />OF NORTH OLMSTED, CUYAHOGA COUNTY, AND STATE OF OHIO: <br />SECTION 1: That the Mayor be and he is hereby authorized, subject to approval <br />by the Board of Control, to enter into a contract with Union Eye Care Center, Inc., to <br />provide eye care and vision services to City employees in connection with the City's self- <br />insured health care plan. The contract shall be substantially in the form of the "City of <br />North Olmsted Vision Care Agreement," attached to this Resolution and marked as <br />"Exhibit A." <br />SECTION 2: That the contract shall be for a two-year term effective from <br />January 1, 2007 through December 31, 2008. <br />SECTION 3: That this Resolution is hereby declared to be an emergency <br />measure immediately necessary for the preservation of the public health, safety and <br />welfare, and for the reason that it is necessary that this Resolution go into effect <br />immediately so that there is no gap in eye care and vision insurance coverage for City <br />employees as of January 1, 2007, and further provided it receives the affirmative vote of <br />two-thirds of all members of Council, it shall take effect and be in force immediately <br />upon its passage and approval by the Mayor.
