No- 311b P- 2 <br />CLEVELAND ELECTRIC ILLUMINATING COMPANY ? ? <br />GENERAL SERVICE INSTALLATION <br />CU9TOMER REQUESTED 68°Ye UP•FRONT PAYMENT OpTION <br />LINE COS?T AGREEMENT <br />This AGREEMENT made this 23 day of AuGUST 2008 is by and between Cleveland Electric Iilumineting <br />Company, herefnafter Gailed the "Company", and the underalgned, herelnafter the "Customer". <br />WITNESSETN: <br />WHEREAS, the Customer desires the Company ro provide new or changed electric service facilities et the <br />premises at 29590 LORAIN RD. NORTHOLMSTED, Ohio, and such facilities will cost the Company SEVEN <br />THOUSAND FIVE HUNDRED EIGHTY SEVEN ($7,5887), and <br />NQW THEREFORE, IT IS AGREED between the perties as follows: <br />1. The Customer ahali make an up-front payment to the Company in the sum of THREE THOUSAND <br />7HIRTY FIVE (53,035) (equal to 4096 of the esUmated total Ilne extension cost of the project plus any premium <br />charges) prior to the start of construction by the Company, <br />2, In addltlon to the cherpea described in Parapraph 1, abnve, under the Company`s line extenalon <br />tedff, the Customer is obllgafad to pay 0.5% of the total line axtension cost as a charpe so long aa the <br />Companys dlstNbuUon rate freeze remalns in efFect. The Customer hes vduMaMiy offered to pey an addlUonal <br />25% of the estlmated totai Iane extension cost ($1,896) in eddwon bo the amount set forth In Paragreph 1,above, <br />in Ileu of making the requlrad Customer monthly paymente required under thls Paragraph, The Company hereby <br />accepts thia affer. Therefore, the Customer's up-front payment shall be 65"/0 of the eadmated total line extenslon <br />oost ($4,931 ) of the proJect plus any promium charges, and no monthly charges will be assessed to the <br />Customer for such line extsnalon proJecG <br />3. This Aereement shali be aaslgned by the Customer to any successor In Interest or any entity that <br />becomes the eustvmer of record for electric usage at the Customer's facUltles at the address set forth above. <br />8. Customer expressly acknowledpes and a8rees to the tertns and opnditiona, marked as Attachment A, <br />which are Incorpcuated herein as If fully set forth at thla polnt. <br />8. The Company's line extenslon paliCy is set forth In the Company's tarlffs and approved by the Public <br />Utilitles Commission of Ohlo.
