.W,, .. ? <br />CITY OF NORTH OLMSTED <br />RESOLUTION NO. 2006 - 131 <br />BY: Mayor O'Grady <br />A RESOLUTION AUTHORIZING THE MAYOR TO ENTER INTO A <br />CONTRACT WITH MAXIM HEALTH SYSTEMS, LLC, FOR THE <br />ADMINISTRATION OF INFLUENZA AND PNEUMONIA VACCINES AT <br />THE NORTH OLMSTED SENIOR CENTER. <br />WHEREAS, it is the desire of the Mayor to enter into a contract on behalf of the City of <br />North Olmsted with Maxim Health Systems, LLC, for the administration of influenza and <br />pneumonia vaccines at the North Olmsted Senior Center; and <br />WHEREAS, whereas it is the desire of this Council to authorize the Mayor to enter into <br />said contract in order to obtain said services for the benefit of the senior population; <br />NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE CITY OF <br />NORTH OLMSTED, COUNTY OF CUYAHOGA, AND STATE OF OHIO: <br />SECTION 1: That the Mayor be and he is hereby authorized to enter into a contract with <br />Maxim Health Systems, LLC, for the administration of influenza and pneumonia vaccines at the <br />North Olmsted Senior Center. Said contract shall be substantially in the form of the <br />"Immunization Calendar - Clinic Provided to North Olmsted Senior Center" Agreement, dated <br />May 2, 2006, marked as Exhibit "A" and attached hereto, with only such changes therein as are <br />not substantially adverse to the City and which are approved by the Director of Law. The <br />character of any such changes as not being substantially adverse to the City shall be evidenced <br />conclusively by the execution of the agreement by the Mayor and the approval of the agreement <br />by the Director of Law. <br />SECTION 2: That this Resolution shall take effect and be in force from and after the <br />earliest period provided by law. <br />PASSED: <br />AT ST.• <br />BARBARA L.
