WHEREAS, the district advisory council of the _ Cuyahoga county, Ohio, general health district, at a *n_eeting held March 7,_ 1977 by a majority vote of members representing the toR~nships and villages did vote affirmatively on the question of providing public health services to the city of Nnrth nlmgtar~ ,Ohio, and did authorize the chairman of the district advisory council to enter into a contract with the mayor of Nnrth nl m~tPd F Ohi n for providing public health services therein; and WHEREAS, the council of the city of No h O1 m ~a Ohio , at a meeting held , by majority vote of all members did vote affirmatively on the question of contracting with the district advisory council of the Cuyahoga county general health district for providing public health services to the city of North Olmsted . Ohio ,and did authorize the mayor to enter into a contract with the chairman of the district advisory council of the _C~yahoga county general health district to provide public health services for the city of North 01mst d, Ohio NOW, THEREFORE, pursuant to such authority, Thomas G, Younct on behalf of the district advisory council of the Cuyahoga county general health district and Mavor Ralph E Christman , on behalf of the city of _ North OI-Tnsted, Ohio do agree as follows• The General Health District of Cuyahoga County, Ohio, hereby agrees to provide health services for the city of North Olmsted Ohio ,for the year 1978 These health services will be provided by the District Board of Health of Cuyahoga County and will include all necessary medical, nursing, sanitary, laboratory and such other health services as are required by the Statutes of the State of Ohio. The City of North Olmsted, Ohio hereby agrees, in return for the health services which will be provided by the District Board of Health of Cuyahoga County for the year 1978 _, to pay to the Cuyahoga County General Health District the sum of Fifty Seven Thousand Fifty One_Dollars ($57,051.00) This sum of ..Fifty Seven Thousand Fifty One Dollars ($57,051.00) ,the City of North Olmsted. Ohio hereby directs the Auditor of Cuyahoga County to place to the credit of the District Health Fund of Cuyahoga County; and the Auditor of Cuyahoga County is hereby authorized to deduct said sum of Fifty Seven Thousand Fifty One Dollars ($57,051 00) in equal semi-annual installments of 'Twenty Eight Thousand Five Hundred Twenty Five Dollars and Fifty Cents ($28,525 50) from the regular tax settlement to be made for said City for the year 1978 This contract shall be in full force and effect from January 1st, 19 78 to December 31st, 1978 IN WITNESS WHEREOF, we hereunder subscribe our names.
