Point of Dispensing [POD] Site <br />Written Agreement Between <br />Cuyahoga County Board of Health And <br />City of North Olmsted Service Department Garage <br />This agreement is made and entered into between the City of North Olmsted Service Department Garage <br />located at 5200 Dover Center Rd., North Olmsted, Ohio 44070, (hereinafter POD) and the Cuyahoga <br />County Board of Health, (hereinafter Board), located at 5550 Venture Drive, Parma, Ohio 44130 this <br />day of , 2009. <br />(hereinafter POD Owner) is authorized to permit <br />Name of organization owing POD <br />the Cuyahoga County Board of Health to use POD, grounds, and equipment for mass clinics required to <br />conduct disease prevention and control activities and wishes to cooperate with the Cuyahoga County Board <br />of Health for such purposes. <br />The parties mutually desire to reach an understanding that will result in making the aforesaid facilities of the <br />POD available to the Cuyahoga County Board of Health for the aforesaid use. Now, therefore, it is <br />mutually agreed between the parties as follows: <br />The POD Owner agrees that, after meeting its responsibilities to pupils, parishioners, members, or clients, it <br />will permit, to the extent of its ability and upon request of the Cuyahoga County Board of Health, the use of <br />the physical facilities listed above and equipment by the Cuyahoga County Board of Health within 24 hours <br />of the request and for the time period being requested, for mass clinics for disease prevention and control <br />activities, including: office equipment, telephones, computers, copy machines, fax machines, tables, chairs, <br />desks, cots, wheelchairs, and refrigerators. <br />Prior to occupancy, representatives of both parties shall inspect the site and shall note, on a separate <br />inspection form, any existing damage or defects. Prior to vacating, representatives from both parties shall <br />again inspect the site and shall note, on a separate inspection form, any new damage. The Cuyahoga County <br />Board of Health agrees that it shall exercise reasonable care in the conduct of its activities in such facilities <br />and agrees to replace or reimburse the POD Owner for any supplies that maybe used by Cuyahoga County <br />Board of Health in conducting mass clinics. In addition, no major modifications or permanent changes shall <br />be made to the site except with the express, prior written approval of POD Owner. <br />This agreement is valid until December 31, 2011. <br />IN WITNESS WHEREOF, the parties have executed this Agreement as of the date set forth above. <br />For The City of North Olmsted: <br />Authorized POD Representative <br />Date <br />For The Cuyahoga County Board of Health: <br />Terrence Allan, MPH, Health Commissioner <br />Date <br />
