<br />Cleveland State University <br />Community Service Learning Program <br />Student Agreement and Release <br />Summer 2009 Program <br />As a student interested in participating in the CSU Community Service Learning Program, a program of the <br />Student Employment Office, I understand that: <br />A) Students participating in this program must have been enrolled for not less than 6 credit hours <br />during Spring 2009 and must be enrolled for not less than 6 credit hours during Fa112009. <br />B) My Federal Work Study award for the 2009-2010 academic year can be used beginning May 10, <br />2009, even though the academic term does not begin until August. By using eligibility before the <br />beginning of the Fall academic term, I acknowledge that I am registered for Fall semester 2009 and have <br />every intention of attending. If for any reason I do not enroll for at least 6 credit hours for Fall 2009 I <br />understand that my FWS award will be rescinded and I will reimburse Cleveland State University for all <br />wages earned from May 17 2009 through the date of the termination of my employment in the CSU <br />Community Service Learning_Program. <br />C) It is my responsibility to understand and abide by the policies of the employing agency, as well as <br />those governing the Student Employment program. These include but are not limited to: <br />• Student employees must notify the Student Employment Office of a job offer by an Agency and <br />get affirmation from the Student Employment Office that their position is being funded; <br />• Student employees must be enrolled for at least 6 credit hours during the regular academic <br />semesters (Fall and Spring); <br />• Student employees may not work more than 20 hours per week, during active terms of enrollment; <br />• Student employees may not work during their scheduled class times; <br />• FWS cannot be used to pay for more than 40 hours per week during breaks, including summer, nor <br />for more than 8 hours of workaday; <br />• Student employees must communicate with the Student Employment Office and promptly respond <br />to requests for information. <br />D) It may be necessary for CSU to provide information to employers participating in the CSU <br />Community Service Learning Program. This information may include but will not necessarily be limited <br />to: Federal Work Study award, CSU identification, Social Security number, credits of enrollment, and <br />grade point average.
