<br />the application. It is further agreed that the County will review the map for accuracy and <br />require it to be re-worked in whole or in part if sufficient inaccuracies exist. <br />B. One helicopter larviciding application will be made to 750 Acres at $62.10 per <br />acre with Biosid. <br />Total: 750 Acres x $62.10/Acre= $46,575.00 <br />Biosid is an environmentally sensitive formulation consisting of the active ingredient of <br />methoprene (0.1 %). Biosid is an insect growth regulator. Biosid granules will provide a <br />14-day residual. Biosid controls on contact and does not need to be ingested. <br />See enclosed map for boundaries and acreage. The Biosid granules will be applied at a <br />rate of 10 pounds per acre achieving a 75-foot swath width at an altitude of l0U feet. The <br />Global Positioning System (GPS), a precise system of navigation, is employed to keep <br />insecticide placement on target. <br />The application includes all labor, equipment, materials and products to be applied. The <br />County retains the right to change areas to be treated provided that Clarke will be <br />afforded adequate notice by the County of any changes. The application will be made in <br />April or May depending on larval activity and authorization by the County. Clarke will <br />be able to apply treatment within seventy-two (72) hours of notification. <br />C. Clarke certifies that it is or will be fully licensed for the required categories of <br />insecticide application in the State if Ohio. Clarke further certifies that it is experienced <br />and knowledgeable in the identification, mapping and treatment of mosquito breeding <br />grounds with granular larvicide and that we have performed such work for at lease one <br />season for a governmental entity. Clarke has furnished references as requested. <br />Clarke further states that all equipment and personnel are in their direct control and <br />employment and that subcontractors will not be used. <br />Within the limitations imposed by adverse weather that would prevent aerial insecticide <br />application, Clarke warrants that its techniques and chemicals will be effective within <br />reasonable bounds in preventing adult mosquitoes from hatching. If Clarke fails to <br />provide effective larval control or fails to make the insecticide application within the <br />agreed period of time due to equipment failure, ineffective chemicals, ineffective <br />application techniques or negligence in performing any part of the work, Clarke certifies <br />to the County that it will provide repeat larval treatments to necessary areas at no <br />additional cost to the Cuyahoga County Board of Health. The intent of this provision is <br />to provide warranty on the work performed. <br />
