Olmsted, Oh. 44070-3129 re: JAT File 0I13
North Olmsted Rec. Ctr.
att: Paul Deichmann, City Engineer Entrance & Toilet Rooms Renovations
For ADA Accessibility
Mr. Deichmann:
Thank you for the invitation to submit a proposal for referenced project.
Based on our conversation regarding format of the drawings ans specifications,
which you agreed can all be on the drawings for this type project, and based
on our visit to the building last Friday, the following is our
PROPOSAL FOR PROFESSIONAL SERVICES
A FIXED FEE OF --------------------------------------------- $ 4,500.00
The above includes:
1. Our time and that of our mechanical and electrical consultants.
2. Our time "in-the-field" to obtain pertinent data for existing conditions.
3. Our time to review design concepts with the City, and to obtain any
existing drawings.
4. Preparation of final Bid Documents, including by reference AIA General
Conditiors, and if needed, aur version of a Bid Proposal format.
5. A preliminary cost estimate far the work.
6. Our time to assist during bidding. The above does not include:
l. Costs of printing the drawings for bid and construction. If requested we will
obtain as many sets as you request, and bill the City at cost plus 5%.
2. Costs to bind all documents into a project manual. Per Mr. Deichmann the
City would prepare the manual, with all "boilerplate" and bid forms.
3. Drawings and specs in our office are not prepared electronically, I trust
for this type project this is not a problem for the City.
Project parameters would be as follows:
A- Our Consultants would be l- Fred Holman, PE for plumbing and ventilation.
2- Charles A. Lewis & Associates, for electrical.
B- Production time would be a maximum of four weeks, excluding City reviews.
