Employees involved in an accident involving a City vehicle, equipment or other <br />property sha11 notify their supervisor as soon as possible. The empioyee shall complete a poiice <br />report and all other report forms required by the employer. <br />Section 6. There shall be a Labor Management Committee established, consisting of three (3) <br />persons from the union and three (3) persons from management. Meetings will be held quarterly or <br />as needed by mutual agreement of the parties to promote a drug free workplace and/or other <br />subjects of mutual interest to the parties. Meetings shall be heid during the last thirty (30) minutes <br />of the shift. <br />ARTICLE 28 - DISABLED EMPLOYEE <br />1 <br />Section 1. The City and the Union shall attempt through agreement to place partially disabled <br />empioyees on light-work jobs which are available or become available during the term of their <br />disability, and which they are qualified to perform. <br />Section 2. The City will require that any employee who is disabled off the job, and is absent from <br />work (sick #ime or leave of absence) for a period in excess of 3 days will be required to submit a City <br />fumished Retum to Work Form. An employee injured on the job. shall provide a City furnished Retum <br />to Work Form, prior to returning to work. in lieu of a City furnished Return to Work Form, an <br />employee may provide any simi{ar form, or attending physician's written statement, which specifies <br />either the ability to return to work'wvithout limitations" or " limitations" exist from date to date. <br />Section 3. Once a disabled employee has exhausted his sick leave benefits, the City wiil <br />continue to pay its portion of his paid hospitalization for 90 days. <br />Seetion 4.
