<br />THIArz,=-, INDEPENDENT CONTRACTOR AGREEMENT is entered into this ? <br />? - day of <br />1998, at North Olmsted, Ohio, by and between the EITY OF NORTH <br />O MSTED, OHIO, an Ohio municipal corporation [hereinafter referred to as "the CITY"], <br />located at 5200 Dover Center Road, North Olmsted, Ohio, and ROBERT J. KRYNAK, whose <br />address is 24140 Gessner Road, North Olmsted, Ohio 44070 [hereinafter referred to as "the <br />CONTRACTOR"]. <br />CONTRACTOR agrees to perform traffic signal services at the date, time and place <br />designated by the CITY. CONTRACTOR will determine the method, details and means of <br />performing said services, and the CITY will not exercise control over such performance, but will <br />merely specify the desired results. Said traffic signal services will include, but not be limitai to, <br />servicing tcaffic signals within the closed loop system and providing consulting services regarding <br />traffic signalizarion. <br />The CITY agrees that, whiie CONTRACTOR is performing said services for the CITY, <br />the CITY shall allow the CONTRACTOR to have access to the City Hall computer and to the City <br />Garage (Traffic Signal room), and further shall provide CONTRACTOR with the use of the City's <br />traffic signal truck. The CITY further agrees that it shall anange for CONTRACTOR to be added <br />to the City's liability insurance policy as an additional insured in order to provide CONTRACTOR <br />with liability insurance coverage while he is performing services within the scope of this <br />Agreement. <br />CONTRAC'TOR shall be compensated by the CITY on an hourly basis for services <br />performed, at the rate of twenty-nine dollars ($29.00) per hour. CONTRACTOR shall invoice <br />the CITY monthly for services rendered. CONTRACTOR shall devote such time to the <br />performance of his duties under this Agreement as is reasonably necessary for the satisfactory <br />performance of the same. <br />CONTRACTOR hereby acknowledges that CITY will not be and is under no obligation <br />to provide CONTRACTOR with any health insurance, workers' compensation coverage, <br />unemployment compensation coverage, or any other type or lcind of insurance and/or other <br />benefit, whether private vr governmental, which is not expressly provided for in this Agreement. <br />
