? <br />r . <br />SIIPPLEMENT TO INSIIR.ANCE <br />ARTICLE %%%I ? <br />INSURANCE <br />Section 1: The City shall provide either individual or family <br />medical insurance coverage, at the employees option, for each full- <br />time employee. The Citp and the Association, in accordance with the <br />plan specified below (Section 3), shall have the option of seeking <br />alternate health insurance providers or plans and will attempt to <br />offer at least two different plans. Basic hospitalization will be <br />consistent with and similar to policies/plans offered on December I, <br />1991. <br />A. Period of Januar I- June 30, 1992 <br />1) Effective during January 1-July 31, 1992, the enployer <br />payment for all medical, hospital, prescription drug and <br />related fees shall be $425.00 per month. A paproll <br />deduction of $15.00 per month shall be made for aIl <br />employees in plans which have an expected cost or rate in <br />excess of $425.00 per month. The employee contribution of <br />$15.00 for the month of July 1992, shall be paid in full <br />without proration. <br />2) If the average cost or prenium is established to be <br />less than $440.00 per month over the above seven month <br />period, the employee who paid with a payroll contribution <br />shall be entitled to a refund determined by the difference <br />between $440.00 and the actual average cost or premium <br />times (7) seven months, but in no event greater than that <br />employee's contributions during the period specified above. <br />All newly hired employees must have participated with a <br />payroll deduction during the entire (7) seven months to be <br />eligible for a refund. Any employee who changes from <br />single to family status or vice-versa during the above <br />period shall be entitled to a refund calculated only by the <br />number of months for which contribution was made. <br />3) If the average cost per month should exceed $440.00 per <br />month for the seven nonth period, the additional amount <br />shall be paid at a ratio of two-thirds by the City and <br />one-third by the employee. <br />- Employee's who may be entitled to a refund shall receive <br />such refund within (30) thirty day's following the determination of <br />the'actual average cost for the period, January I, 1992 through July <br />31, 1992.
