? <br />Page 2 <br />City of North Olmsted <br />? Attn: Mr. Ralph Bohlmann, <br />Service Director <br />1. SCOPE OF SERVICES <br />A. Manhole Inspection <br /> <br />January 15, 1996 <br />? The W. R. E. S. Corporation (a. k. a. The Western Reserve Engineering and Surveying <br />Company) will provide all field inspection necessary to complete the manhole inspection form. <br />The inspection team is currently certified in Confined Space Entry requirements, as evidenced <br />? by the enclosed information. Manhole entry forms will also be completed for all manholes to <br />ensure proper safety procedures are followed. <br />? In addition to the manhole inspection form, a color video tape (VHS format) will be made <br />for each manhole to provide a record of the condition of the manhole, including any deficiencies. <br />We feel that his will enable us to make an accurate determination of the necessary repairs of <br />? deficient manholes. The tapes will be provided to the City. <br />B. Manhole Evaluation <br />? A tabulation of the manholes which are inspected will be PrePared. The tabulation will <br />include the general conditions and construction of the manhole as noted on the inspection forms, <br />? and nature of deficiencies which are found. <br />Manholes which exhibit structural deficiencies or sources of inf'iltration and inflow (I/I) <br />? will be further evaluated and recommendations made for the repairs which should be done to the <br />manholes. An estimated construction cost will be prepared for these recommendations. <br />II. CONFINED SPACE ENTRY OUALIFICATIONS AND INFORMATION <br />? The following items are enclosed to demonstrate W. R. E. S. ability and qualifications for <br />this project. <br />? A. Letter of Recommendation from CTE Engineers <br />B. Western Reserve Engineering and Surveying Company Environmental Occupational <br />? Health and Safety Manual <br />C. WRES Sewer Entry Procedure <br />? D. WRES Manhole Inspection Form Instructions with examples of Inspection Form and <br />Entry Permit <br />? <br />
