BASIS AND METHOD OF PAYMENT <br /> • Disposal Weights: The contractor must provide a scale for determining the total weight of the <br /> biosolids disposed of. <br /> • Hauling Rates: For transportation and disposal services required to be performed,the charges <br /> shall not exceed the unit prices as fixed by the Contract Documents. <br /> • Tickets:The Contractor shall complete and provide the Wastewater Division with a numbered <br /> hauling ticket prior to the Contractor leaving the North Olmsted Wastewater Treatment Plant <br /> with one of the numbered 23-yard containers.The hauling ticket shall be in a format acceptable <br /> to the City. The hauling ticket shall identify at a minimum the number of the 23-yard container <br /> being transported, the date of the transport, the driver of the vehicle used for the <br /> transportation activity, and the Contractor's number for the vehicle beings used for the <br /> transportation activity. <br /> • Biosolids Manifests—The Contractor shall complete and provide the Wastewater Division with <br /> a preliminary numbered biosolids manifest prior to the Contractor leaving the North Olmsted <br /> Wastewater Treatment Plant with one of the numbered 23-yard container. The Contractor shall <br /> also provide the Wastewater Division with a completed version of each preliminary numbered <br /> biosolids manifest within ten (10)_working days after the disposal activity has been completed. <br /> The biosolids manifest shall be in a format acceptable to the City.The completed hauling tickets <br /> shall identify the weight of the hauled biosolids in net weight in pounds and wet tons, the <br /> number of the 23-yard container transported, and the date of the transport.. Given the <br /> information that is to be placed on the completed biosolids manifest, the manifest shall be <br /> considered a chain of custody form by the City and the Contractor. <br /> • Monthly Summary Report—The Contractor shall provide the Wastewater Division with a <br /> monthly summary report that identifies the information contained on each biosolids manifest. <br /> The monthly summary report shall be in a format acceptable to the City. The monthly summary <br /> report shall be submitted to the Wastewater Division within 10 working days after the first <br /> working day of the following month. <br /> • Payment-The Contractor shall invoice the City once per month for all transportation and <br /> disposal costs incurred during previous month.The invoice shall be reviewed by Wastewater <br /> Division management personnel prior to the processing of a notice for acceptability to the <br /> Director of Public Safety and Director of Finance. <br /> TRANSFERABILITY OF CONTRACT <br /> No assignment of the Contract or any right accruing under this Contract shall be made in whole or in <br /> part by the Contractor without consent of the City. <br /> OWNERSHIP <br /> Title to Wastewater Treatment Plant Biosolids shall pass to the Contractor when placed in the <br /> Contractor's disposal facility stated in the bid, or removed by the Contractor from the customer's <br /> premises, whichever last occurs. <br /> NOWWTP 2014 BIOSOLIDS BID SPECIFICATIONS <br />
