Automated Utilization Report Requirements & Software Systems <br />As part of the set up in the location software for new enrollees, the location will need to input the <br />16 -digit Healthways card number (not their insurance ID) into their software. This 16 -digit <br />number can go into any field that can hold 16 characters (all staff who sign -up new members in <br />the facility must place those numbers in the same field). <br />Important Note: Use one unique ID number per member. Do not report family members or <br />couples under one membership ID number. <br />Reports must only contain the following information in six separate columns: <br />a. Member ID # (your own permanent internal ID for each member). <br />b. 16 digit Healthways ID # <br />c. First name <br />d. Last name <br />e. Date of visit <br />f.
