permit holder and the opportunity for a public hearing thereon before the Mayor of the City of North <br />Olmsted or the Director of Public Safety if acting as the Mayor's designated agent therefor. <br />749.06 APPLICATION FOR MASSEUR OR MASSEUSE LICENSE; EXPIRATION AND CONTENTS. <br />(a) Any individual desiring to perform services as a masseur or masseuse within the City of North <br />Olmsted shall first make application to the City of North Olmsted upon the forms prescribed by the City <br />therefor. Application and fee payment shall be made at the City of North Olmsted, 5200 Dover Center <br />Road, North Olmsted, Ohio 44070. Each such application shall include a filing fee of two hundred fifty <br />dollars ($250.00) which shall be non - refundable. Each application must be completed in its entirety <br />upon being filed with the City of North Olmsted. The completed application shall include the following <br />information: <br />(1) The results of a physical examination performed by a licensed physician within thirty <br />days of the application certifying that the applicant is free from contagious or communicable diseases; <br />(2) The full name, date of birth, weight, height, color of hair and eyes, present address, <br />how long at present address, and Social Security number of the applicant, and the last previous <br />employer of the applicant; <br />(3) Written authorization for an investigation into the criminal record of the individual <br />applying for the license, which will require the applicant to make an appointment with a law <br />enforcement agency of the City of North Olmsted's choosing to be photographed and fingerprinted for <br />the background investigation; <br />(4) That the applicant is at least twenty -one years of age and proper proof thereof (i.e. a <br />certified birth certificate, driver's license, passport or state identification); <br />(5) The applicant's immigration and work authorization status and proper proof thereof, <br />(6) Any other information determined by the City of North Olmsted to be necessary. <br />(b) Upon the filing of the application called for by this section, a representative of the City shall <br />provide to the applicant a copy of this chapter and all necessary information and forms concerning <br />masseurs or masseuses and shall obtain a signed acknowledgment from the applicant acknowledging <br />receipt thereof. <br />(c) Upon the filing of the application called for by this section, the City of North Olmsted shall have <br />an investigation conducted on the information contained on the application. <br />(d) An initial license issued under this section to a masseur or masseuse shall expire one year after <br />the date of issuance of same, except that no masseur or masseuse shall be required to discontinue <br />performing massages because of the failure of the City to act on a renewal application filed in a timely <br />manner and pending before the City on the expiration date of said initial license. <br />
