CCBH 2015 SEMI-ANNUAL REPORT City of North Olmsted Page 8 <br /> PERSONAL RESPONSIBILITY EDUCATION PROGRAM <br /> The PREP (Personal Responsibility Education Program) for Foster Care and <br /> Adjudicated Youth Grant aims to reduce teen pregnancy and sexually transmitted <br /> infection rates for youth who are currently in foster care or the juvenile justice systems <br /> by educating the direct-care staff in those systems to become trainers in evidence- <br /> based pregnancy prevention programming. This train-the-trainer model seeks to <br /> enhance professional development at the local level for direct care staff while <br /> providing sustainability for pregnancy prevention and life skills education for youth in <br /> the target systems. The PREP Grant is a five year initiative serving the following five <br /> counties of Northeast Ohio: Ashtabula, Lake, Geauga, Cuyahoga and Lorain. <br /> Program.Inquiries.and Resp.anses.(in hours)* 25 <br /> HEALTH IMPROVEMENT PARTNERSHIP - CUYAHOGA <br /> Heath improvement Partnership - Cuyahoga is a collaborative, county-wide planning <br /> process focused on developing a community health improvement plan that will serve <br /> as a guide for improving the health status and quality of life for all people who live, <br /> work, learn and play in Cuyahoga County. <br /> Program.lnquiries.and Respanses.(in hours)* 27 <br /> SENIOR FALL PREVENTION PROJECTS <br /> CCBH collaborated on the Senior Safety Project (S.S.P.) with the Cuyahoga County <br /> Department of Development. The SSP offers seniors an opportunity to structurally <br /> modify their home reducing their risk of falling. A fall risk assessment is conducted <br /> with the senior by Department of Development and modifications are based upon the <br /> results of the home assessment. A referral is made to CCBH for a fall risk education <br /> visit to discuss the risk factors associated with personal behaviors. Eligibility <br /> guidelines must be met to qualify. The Home Injury Prevention Project (H.I.P.P.) <br /> offers fall risk education in a congregate setting. <br /> Program.1nquiries.and Responses.(in hours)* 1. <br /> UNUSED PRESCIPTION MEDICATION AWARENESS INITIATIVE <br /> Rates of unintentional fatal poisoning due to prescription medications have risen over <br /> 250% since 1999. The unused prescription medication awareness initiative is a <br /> collaborative approach to educating the public on the factors contributing to this new <br /> epidemic and informing the public on the proper ways to dispose of unused <br /> medications. <br />
