CCBH 2016 SEMI-ANNUAL REPORT City of North Olmsted Page 2 <br />NEWBORN HOME VISITING: WELCOME HOME <br />Mothers that are 200% or greater of the Federal Poverty Guidelines are eligible to <br />receive home visits from a public health nurse. Also eligible for a visit are all teen <br />moms and any mom with an identified medical or social risk factor. The nurse will <br />discuss basic newborn care, home safety, parenting, feeding, and concerns of the <br />parent(s). Additionally, the visit will consist of an assessment of the baby, and will <br />provide the parent(s) with resources and make referrals to community agencies as <br />needed. <br />Program.lnquiri.es.and Responses.(in hours).*......................30 <br />Visits......................................................6 <br />Case. Management...........................................1 <br />LEAD POISONING PREVENTION <br />Public health nurses provide lead screening, education, and case management. <br />Public health sanitarians perform investigations in response to reports of elevated <br />blood lead levels in children to identify potential sources of lead exposure. Lead risk <br />assessments, housing remediation, and clearance testing are conducted as part of <br />collaboration with the Cuyahoga County Department of Development and our <br />municipalities, through a grant with the U.S. Department of Housing & Urban <br />Development. <br />Program.lnquiries.and Respanses.(in hours)*.......................3 <br />Case. Management ...........................................2 <br />CHILDREN WITH MEDICAL HANDICAPS (CMH) <br />CMH is a state and federally funded health care program of the Ohio Department of <br />Health. Serving as a safety -net insurance program and payer of last resort, CMH <br />links children with special health care needs, and their families, to services that can <br />diagnose a possible medical condition (Diagnostic Program). It also assists in <br />obtaining payment for ongoing services their child might need (Treatment Program). <br />A child must have a qualifying medical condition to be on the diagnostic and <br />treatment programs and must see a CMH approved provider annually. The <br />Treatment program also has income requirements. A child may be eligible from birth <br />to 21 years of age and the Treatment program must be renewed annually. <br />
