Rev 5/9/18 <br />(d) Evidence of Compliance. All applications for local license and registration <br />certificate shall include the following documentation, to be supplied by the applicant: <br />(1) A certificate of insurance for general liability for bodily injury and <br />property damage in the amount of $500,000.00; <br />(2) A certificate of insurance for construction claims and unworkmanlike <br />performance in the amount of $500,000.00; <br />(3) A certificate of coverage from the Bureau of Workers' Compensation; and <br />(4) Federal tax or payroll identification number or Regional Income Tax <br />Account number or other evidence of registration and/or certification of <br />compliance with RITA withholdina. <br />(e) Issuance of Certificate. The Building Commissioner, upon receipt of all required <br />fees, documentation and evidence of compliance necessary to satisfy the provisions herein, shall <br />issue or deny a certificate of registration. An application shall not be deemed to have been <br />approved until the Building Commissioner has examined such application, determined therefrom <br />that the applicant has satisfied the requirements of Section 1317.04, and has issued a certificate <br />of registration. <br />(f) Appeals. The applicant for a registration certificate may appeal from the denial <br />by the Building Commissioner to the Building and Zoning Board of Appeals. The Board shall <br />hear and decide the appeal at its next regular meeting. At the discretion of the Building and <br />Zoning Board of Appeals, after a review of records and an oral examination, a temporary <br />registration certificate may be issued but not to exceed two years in duration. After the two-year <br />probationary period, the applicant may reapply to the Building Commissioner and, upon appeal, <br />the Board shall either deny or affirm the application. <br />(g) Records. The Building Commissioner shall keep a record of all certificates of <br />registration and certificates for local licenses, and together with other documentation forming the <br />basis for the issuance, or the denial thereof, on file for not less than three years. <br />1317.05 GENERAL QUALIFICATIONS. <br />(a) Notwithstanding an applicant's compliance with Section 1317.04, no contractors <br />shall be issued a registration certificate, nor shall such registration be renewed, if the Building <br />Commissioner is in possession of evidence that the applicant lacks fitness to perform the <br />work . In any determination as to the V,,er-a' <br />fitness to perform the work of an applicant, the Building Commissioner shall consider the <br />applicant's record of violations of the Building Code, of other laws or ordinances regulating <br />building construction in the City, the causes listed in Section 1317.06(a) and evidence of <br />similar violations in other communities. Evidence of dishonest construction practices or <br />malpractice in the conduct of a business, trade or profession, shall also be considered as evidence <br />of m ' h t the lack thereef a lack of fitness to perform the work. Upon receipt and <br />review of such evidence, the Building Commissioner may determine that it is in the public <br />interest that such applicant not be issued a certificate of registration or that same not be renewed <br />and thereby refuse or revoke authority to engage in work or practice a trade in this City.
