. ?.. ?., <br />? . ,.?..,? <br />?- <br />, ? !?;- - .?..?. <br />. t <br />.; <br />, <br />CITY OF NORTH OLMSTED, OHIO <br />ORDINANCE N0. 82- 154 <br />PAGE 4. <br />(l) The name and address, age and birth date of <br />the applicant. <br />(2) The location where the mechanical or electrical <br />amusement devices are to be displayed or operated <br />and the business conducted therein. <br />(3) A description of the machine or machines to be <br />covered by the license, its mechanical features, <br />the name of the manufacturer, and the serial number <br />of the said machine. <br />(4) The name o£ the owner or owners of the premises <br />wherein the devices are to be displayed or <br />operated. <br />(5) The name of the owner or owners of the business <br />conducted therein. <br />(6) The name of the owner o£ the machines, if the <br />person is different than the applicant. <br />(7) Whether or not the applicant or the owner of the <br />machines has been convicted of a gambling offense <br />or for the sale or conviction of a drug offense <br />in the last ten (10) years prior to the date of <br />application. <br />SECTION 3: That any ordinance or parts of ordinances which <br />are in con lict with the foregoing provisions be and the same are <br />hereby repealed upon the ef£ective date of this Ordinance. <br />SECTION 4: That this Ordinance is hereby declared to be an <br />emergency measure immediately necessary for the preservation of <br />the public health, safety and welfare and for the further reason <br />that it is immediately necessary to protect the public health, <br />safety and welfare with respect to regulation of game rooms and <br />also that the new regulations will be effective on January l, 1983, <br />and further provided it receives an affirmative vote of two-thirds <br />of all members elected to Council, it shall take effect and be in <br />force immediately upon its passage and approval by the Mayor. <br />PAS S E D: <br />ATTEST: <br />FLORENCE CAMPBELL, LERK OF COUNCIL <br />? ??'?.c.l._ <br />(2' <br />AMES D.
