CONTRACT FOR PUBLIC HEALTH SERVICES (City with a General Health District) (Authority-Sec. 3709.08 R.C.) By WHEREAS, the disuict advisory council of the Cuyahoga county, Ohio, general health district, at ? *+?eeting held Plarch 23, 1982 , by a majority vote of inembers representing the townships and villa es did vote affirmatively oa the question of providing public health setvices to the city of Nort? Olmsted Ohio, and did authorize the chairman of the district advisory couacil to enter into a contract with the mayor of North Olmsted, Ohio for providiag public health services therein; and WHEREAS, the couacil of the city of North Olmsted, Ohio , at a meeting held , by majority vote of all members did vote affirmatively on the question of coatracting with the district advisory council of the Cliyrlhoga county general health district for providing public health services to the city of North Olmsted, Ohio , and did suthorize the mayor to enter into a contract with the chairman of the district advisory council of the Cuyahoga couaty general health district to provide public health setvices for the ciry of North Olmsted, Ohio , NOW, THEREFORE, pursuant to such authority, Mavor Robert D. Cusick on behalf of the district advisory council of the Cuvahoga county general health district and Mavor Robert W. Swietvniowski , on behalf of the city of North Olmsted Ohio do agree as follows: The Geaeral Health Disuict of Cuyahoga County, Ohio, hereby agrees to provide health services for the city of North Olmsted, Ohio , for the year 1983 . These health services will be provided by che District Board of Health of Cuyahoga County and will include all necessary medical, aursing, sanitary, laboratory and such other health setvices as are required by the Statutes of the State of Ohio. The City of North Olmsted, Ohio herebq agreea, in recurn for the health eervicea which will be provided by the Discrict Board of Health of Cuyahoga Couaty for the year 1983 to pay to the Cuyahoga County General Health Disuict the sum of Fifty Five Thousand Four Hundred Fift,v Nine Dollars ($55.459.00) This sum of Fifty Five Thousand Four Hundred Fifty Nine pollars ($55,459.00) , the City of North Olmsted, Ohio hereby direccs the Auditor of Cuyahoga County to place to the credit of che Distria Health Fuad of Cuyahoga County; and the Audicor of Cuyahoga County is hereby suthorized to deduct said sum of Fiftv Five Thousand Four Hundred Fifty Nine Dollars ($55 459.00) in equal semi-annual installments of Twentv Seven Thousand Seven Hundred Taentv Nine Dollars and Fifty Cents ($27,729.50) from the regular tax settlement to be made for said City for the year 1983 . This contract shall be in full force and effect from Jaauary lst, 19 83 to December 31st, 150-3 . IN WITNESS WHEREOF, we hereunder subscribe our names.
