CITY OF NORTH OLMS"TLD <br />RESOLUTION NO. 2019 - 62 <br />BY: Mayor Kennedy <br />A RESOLUTION AUTHORIZING THE DIRECTOR OF HUMAN <br />RESOURCES TO SOLICIT PROPOSALS FROM HEALTH CARE <br />PROVIDER'S FOR THE VISION COMPONENT OF THE CITY'S <br />HEALTH CARE & HOSPITALIZATION PLANS FOR 2019, AND <br />FURTHER AUTHORIZING THE IVIA YOR, SUBJECT TO <br />APPROV4L OF THE BOARD OF CONTROL, TO ENTER INTO A <br />CONTRACT FOR HEALTH SERVICES, AND DECLARING AN <br />EMERGENCY. <br />WHEREAS, the specifications for Requests for Proposals for a personal services <br />contract with a qualified health care provider to cover the vision and eye care component <br />of the City's f lealth Care Coverage/Hospitalization Plans are on file in the Office of the <br />Director of Human Resources; and <br />WHEREAS, it is the desire of this Council to authorize the Director of Human <br />Resources to solicit proposals for said personal services contract and ancillary vision care <br />services, and to further authorize the Mayor, upon approval of the Hoard of Control, to <br />enter into an agreement or agreements with the most qualified vision care provider to <br />perform the services and provide the umbrella insurance at a price that is fair and <br />reasonable to the City. <br />NOW THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE CITY <br />OF NORTH OLMSTED, COUNTY OF CUYAHOGA AND STATE OF OHIO: <br />SECTION 1: That the Director of Human Resources be and she is hereby <br />authorized to solicit or request proposals for a personal services contract between the City <br />of North Olmsted and a qualified vision care provider to Supplement the City's Health <br />Care Coverage/Hospitalization Plans, in accordance with the specifications now on file in <br />her office and that, upon approval by the Hoard of Control, that the Mayor is hereby <br />authorized to enter into an agreement or agreements for said vision care services from the <br />most qualified firm at a price that is fair and reasonable to the City. <br />SECTION 2: That this Resolution is hereby declared to be an emergency <br />measure immediately necessary for the preservation of the public health, safety and <br />welfare and for the further reason that it is immediately necessary that this legislation go <br />into effect immediately because Union Eye Care defaulted on its agreement to provide <br />vision care to City employees and replacement of such coverage is urgent; and further <br />provided it receives the affirmative vote of two-thirds of all members of Council, it shall <br />take effect and be in force immediately upon its passage and approval by the Mayor. <br />
