www.ohiofoodbanks.org Fresh Food Capacity Building Grant Agreement SFY 2020 Grant Agreement between the Ohio Association of Foodbanks and North Olmsted Oxcart Pantry The Ohio Association of Foodbanks (hereinafter referred to as the association), whose address is 100 East Broad Street, Suite 501, Columbus, OH 43215, and North Olmsted Oxcart Pantry (hereinafter referred to as Grantee), whose address is 26273 Butternut Ridge Road, North Olmsted, OH 44070, hereby enter into this agreement. The association and Grantee in consideration of the mutual promises expressed below and intending to be legally bound agree to the following provisions. Responsibilities of the association A. The association, a 501(c)(3) organization, agrees to provide a grant in the amount of $5000.00 to the Grantee, a tax exempt organization, to coordinate activities as described in paragraph A of the section titled Responsibilities of Grantee. B. The grant will be provided for the period of August 18, 2019 through June 30, 2020. C. Funds will be distributed on a one-time reimbursement basis with the submission and documentation of equipment purchases being completed. D. The association reserves the right to require immediate return of funds/equipment in the event that Grantee fails to use the funds for the purposes for which they were provide or the Grantee fails to fulfill its obligations. Responsibilities of Grantee A. Grantee will utilize purchased equipment to increase and maintain access to nutritious, fresh foods for low-income Ohio households. Grantee will be reimbursed on a one-time basis following submission of receipts/documentation of approved purchases. Grantee will agree to maintain good standing with their respective Feeding America foodbank throughout the duration of the grant period and at least three years after the end of the grant period. Grantee further agrees to not use the grant funds for any purpose prohibited by law or that would jeopardize the legal or tax status of the association. B. Additionally, Grantee agrees to maintain its books and records to show and account for the funds received under this agreement, to maintain records to identify the purposes for which the funds have been used, and to retain such records for five years. Grantee agrees to maintain any and all receipts for which the grant funds were utilized for a period of five years and be able to document all expenses for which funds were used. In the case this is not possible, Grantee agrees to provide the association with any supporting documentation that may be necessary and requested. C. Grantee agrees to return the funds and/or equipment to the association if the Grantee discontinues its services and/or discontinues its membership with its respective Feeding America foodbank at any time before June 30, 2023. D. Grantee agrees to complete approved equipment purchases, incur approved expenses, and submit an invoice for reimbursement with documentation and receipts of purchases no later than May 30, 2020. If Grantee has yet to submit an invoice for reimbursement by this due date, this contract may be voided and the Grantee's funds may be reallocated at the discretion of the association. ,to Fresh Food Capacity Building Grant Agreement SFY 2020 OHIO ASSOCIATION OF FOODBANKS
