Internet, Cable and Phone ("Operator" or "WOW!") <br />Legal Name Corporate Office Address <br />7887 E Belleview Ave Ste 1000 <br />Englewood, CO 80111-6007 <br />Property Address Division Address <br />5200 DOVER CENTER RD 10221 Sweet Valley Dr, Valley View <br />North Olmsted, Ohio 44070 OH, 44125 <br />WOW! refers to the operating company subsidiary of WOW! Internet, Cable and Phone that owns and/or operates the broadband system where the <br />Property is located. <br />Effective Date: <br />The Owner of the property referenced above (the "Property"), hereby consents to the installation, operation, maintenance, repair and removal by <br />WOW! at WOW!'s sole cost and expense, of WOW! cable and other equipment (the "System") in, over, under, across and along the Property, to be <br />owned and used solely by WOW! to provide communication services to tenants and other occupants of the Property. <br />WOW! shall repair any damage to the Property caused by the installation, operation or maintenance of WOW!'s equipment on the Property. <br />Owner's consent as described herein will continue for so long as WOW! provides or offers communications services to tenants or other occupants <br />of the Property; Owner may terminate this consent if WOW! has not provided service to a tenant or occupant of the Property for six consecutive <br />months. <br />WOW! will install its equipment in a workmanlike manner and in accordance with industry standards. <br />At the expiration or termination of WOW!'s right to access the Property, WOW! has the right but not the obligation to remove or disable all or any <br />portion of the System. <br />WOW! will indemnify and hold harmless the Owner to the extent of any liability or damage to any person or property attributable to the installation, <br />operation and maintenance of WOW!'s equipment on the Property. <br />Agreed to as of the Effective Date stated above. <br />Property .WOW!
