OLIO HEARING REPRESENTATIVE SERVICE SCHEDULE <br />Sedgwick will provide the following services: <br />1. The Sedgwick docketing department receives Notices of IIearing for all Client hearings <br />scheduled statewide before the Ohio Industrial Commission. Sedgwick will provide Client <br />with a special docket advising Client of its upcoming hearings before the Ohio Bureau of <br />Workers' Compensation and Industrial Commission ("Ilearings"). <br />2. Sedgwick will assign a hearing representative ("Hearing Representative") to attend and <br />present Client's position at all of Client's scheduled Hearings. Sedgwick will not assign a <br />hearing representative to those hearings for which Client has advised Sedgwick that Client <br />has retained legal counsel. <br />3. The Hearing Representative will review claim documents in the possession of Sedgwick as <br />well as those materials made available by the Ohio Bureau of Workers' Compensation and <br />the Ohio Industrial Commission. <br />4. The Hearing Representative will also review and be familiar with any case materials <br />provided by the Client. <br />5. The Hearing Representative will review the Hearing Defense Plan and all relevant electronic <br />claim notes entered by the Sedgwick claims examiners. <br />6. The Hearing Representative will present the Client's position at the Hearings and enter a <br />post -hearing note in the electronic claims system to advise what happened at the hearing. <br />The fee for this service is itemized in the attached Exhibit B. <br />C/20912 24 <br />
