EXHIBIT A <br />22. Explain how the business will use these grant funds. Indicate specific expenses such as payroll, rent, <br />mortgage, utilities, equipment leases, insurance, or other expenses. <br />A. Each applicant for funds will include in its application an itemization of the expenses it intends to pay <br />with the grant funds, which will be reviewed for compliance with the eligible expenses set forth. <br />B. Following the expenditure of all grant funds, the business shall provide a listing of all business <br />expenses paid using the grant funds along with supporting documentation. Documentation will be in the <br />form of cancelled checks, paid invoices, bank statements or similar documents evidencing payment. The <br />listing of business expenses paid and accompanying documentation shall be submitted no later than 180 <br />days following the receipt of grant funds by the business. Grantees who do not provide these <br />documents risk future funding from the City of North Olmsted. <br />C. All expenses reported as paid using Program funds must either be incurred by the business or paid by <br />the business within ninety (90) days of receipt of grant funds. No grant funds may be used to pay for <br />vehicles or equipment leased or purchased after the date that the application was submitted. <br />ADDITIONAL DOCUMENTATION: All applicants must submit: (i) a copy of the existing lease (or <br />mortgage); (ii) evidence that the business is current and in good standing on payment of rent (or <br />mortgage) prior to submitting the application; (iii)evidence of expenses sought to be paid or reimbursed <br />under the Program; and (iii iv) Completed Form W-9. <br />Applications will not be considered until all documents have been received. <br />The undersigned certifies that he/she is authorized to complete, sign and submit this application on <br />behalf of the applicant/owner. Further, the undersigned certifies that the information contained in this <br />application has been reviewed by him/her and that all information, including additional documents, are, <br />to the best of his/her knowledge, complete and accurate and presents fairly the condition of the <br />applicant and project accurately. Intentionally falsifying information in this document constitutes a <br />criminal offense. The undersigned understands that additional information may be required to finalize <br />the approval process. The undersigned also understands that the submission of the application for <br />financial assistance does not automatically constitute approval.
