ARTICLE 4 UNION SECURITY <br />4.01. The Employer shall deduct the initiation fee and regular monthly dues from the pay of <br />employees covered by this Agreement upon receipt from the Union of individual written <br />authorization cards voluntarily executed by an employee for that purpose and bearing his signature. <br />4.02. Deductions under this Article 4 shall be made during the second pay period of each month <br />via electronic ACH Transfer payment into the commercial bank account of Ohio Council 8, <br />AFSCME, AFL-CIO no later than ten (10) days following the end of the pay period in which <br />the deduction is made. The union shall provide the Employer with authorization to make <br />deposits into the financial institution utilized by the Union along with the routing number and <br />account number of the Union's account. It is the Union's responsibility to notify the Employer <br />in writing of any changes to the Union's account information. met If an employee's pay for that <br />period is insufficient to cover Union dues, the Employer will make a deduction from the pay earned <br />during the next period or a subsequent period. <br />of all fil"ds Aedoowd <br />The Employer shall email, with each deduction and transmittal of dues/fees, thefollowing lists <br />of information in Excel format to oc8dues6i)0lscme&orp, subject tine: Local 2681-3, Pay date <br />1. Dues List: (last name, first name, middle initial), current address, phone number, <br />department/work unit, last jour digits ofsocial security number and the amount of the deduction <br />for each employee, as well as, the total amount of the dues deducted for all employees for the <br />pay period of the report. <br />Z Non-member list: In alpha order by last name. The current name, address, phone <br />number departmendwork unit of each bargaining unit employee who are non-members. <br />3. Dropped Employees: In alpha order by last name. The current name, address, phone <br />number of bargaining unit employees who are dropped from the previous dues list and the <br />reason was dropped <br />4. Total Remittance Amount. <br />This Section shall be deemed complied with if one list containing fields with all of the above <br />information (including a field showing whether an employee is a union member or non- <br />member) is provided by the Employer to the Union in Excel for all bargaining unit employees. <br />The grouping of members and non-members, and the total required under this Section shall be <br />calculated and/or organized by the Union using fields provided in the Excel format. <br />4.03. Indemnification.
