The De[~artment, active in cases where it is unaware of the Agency's involver, ent with the client, upon ~3ecoming aware of the A~enc.;- client involvement and before di~Fosition of its re,crt, shall a~vise ~.., ~hone the Agenc,; witkin three (3) days of i~s findings and any recommendaticns for on-going activities and services. IV. The Agency agrees to perform the following duties: A. initiate an assessment on nonmemergency reports of abuse, neglect or exp!citation which are as- signed by the Department within three (3) ~ays of the date of the assignment. Z. Provide, at the time of the initial interview, a ~tritten notice of intent to the client (with a ccpy tc the Department) in language that is reascnably un- derstandable by the subject of the invgstigation. C. Upon completion of the assessment the Agency shall determine whether the Client is in need of on-going protective services and re~rt this together with the Agency's c~plete findings to the Department within thirty (30) days of assisgnment. This report will include at a minimum: 1. Date of assignment. 2. Nature of the complaint. 3. N~es of ~ople ~ontacted. 4, Dates of contact. ~'~' Assessment 6. Dis~si tion (substantiated/unsubstantiated). 7. ~-going plan. V. The Depar~ent and the Agency agree to designate liaisons whose roles will be to: A. Refer re~rts of abuse, neglect or exploitation-; determine if the complaint should be assigned the designated Agency and/or Depar~ent.
