n. Ail industrial users shall promptly notify the City <br />Engineer in advance of any substantial changes in the volume <br />or character of pollutants in their discharge including the <br />listed or characteristic hazardous waste for which the <br />industrial user has submitted initial notification under 40 <br />CFR 403.12 (p)~ <br /> <br /> Section 10. That existing Section 913.10 of <br />Ordinances of the City of Lakewood be amended by <br />additional paragraphs to read as follows. <br /> <br /> the Codified <br />enactment of <br /> <br />913.10 INDUSTRIAL USER REPORTS. <br /> <br /> Compliance Date Report. Within ninety (90) days after <br />the ~date for final compliance by the user with applicable <br />pretreatment standards set forth in these regulations or <br />ninety (90) days following commencement of the introduction <br />of wastewater into the City's sewer system by a new user, any <br />user shall submit to the City a report indicating the nature <br />and concentration of all prohibited or.required substances <br />contained in its discharge, and the average and maximum daily <br />flow in gallons. The report shall state whether the <br />applicable pretreatment standards or requirements are being <br />met on a consistent basis and, if not, it is necessary to <br />bring the user into compliance with the applicable <br />pretreatment standards or requirements. <br /> <br /> Signatory Requirements. Any reports required in this <br />code or regulations shall include the certification statement <br />as follows: <br /> <br />I certify under penalty of law that this document and <br />all attached documents were prepared under my direction <br />or supervision in accordance with a system designed to <br />assure that qualified personnel properly gather and <br />evaluate the information submitted. Based on my inquiry <br />of the person or persons who manage the system or those <br />persons directly responsible for gathering the <br />information submitted is to the best of my knowledge and <br />belief true, accurate ~nd complete. I am aware that <br />there are 'significant penalties for submitting· false <br />information including the possibility of fine and <br />imprisonment for knowing violations. <br /> <br />Further, all reports shall be signed <br />representative of the industrial user. <br /> <br />by an authorized <br /> <br /> Section 11.
