Flaesgarten <br />Firefighter Grade One <br />· Water Rescue Specialist <br />Lakewood Fire Department <br />1418 Warren Road <br />Lakewood, OH 44107. <br /> <br />"EXHIBIT A" <br /> <br />OHIO DEPARTMENT OF <br /> <br />NATURAL RESOURCES <br /> <br />Fountain Square <br />Columbus, Ohio 43224 <br />February 27, 1992 <br /> <br />Dear Mr. Flaesgarten, <br /> <br /> On behalf of Governor Voinovich and the Ohio Department of Natural Resources, Division of Watercu-aft, I am <br />happy to annOUnce the approval of a boating safety education grant to the City of Lakewood Fire Department in the <br />amount of $10,000,00. This grant is to help finmuce your 1992 boating safety education program entitled: "Safety <br />and Rescue In and About Lake Erie and Its Tributaries." <br /> <br /> The approved expenses are indicated in the enclosed copy of your revised estimated budget.' Please note, we <br />have added training and travel money to allow a representative(s)from your agency to attend the required <br />training sessions as described below. Please call us should you need any clarification on any of the changes. <br />~ copies of the grant agreement can be found in your education grant gttidelines packet. Please have both <br />copies signed by officials from your agency and return both copies to the Division of Watercraft. Along with the <br />two copies of the agreement, please also send an ordinance/resolution frc~ the City of Lakewood authorizing your <br />agency to enter into agreement with the Division of Watercraft. We also must have your federal tax <br />identification number before a check can be issued. These are due back to the Division of Watercraft as quickly <br />as possible as no money will be made available prior to the execution of the agreement. <br /> <br /> Do not make any expenditures towards your program at this time. Once the agreement form is cc~pletety <br />executed by officials from your agency and the (bio Department of Natural Resources, purchases can officially be <br />made. we will forward the check to you after the agreement is fully executed. Normally, it takes approximately <br />2-6 weeks for the check to be processed and sent to the grant' recipient once the agreement is signed. Therefore, <br />the quicker we receive the signed agreements, the quicker we can get your check processed. <br /> <br /> Please read the agreement over carefully. Remember, any changes i~ expenditures exceeding 10% of the <br />estimated budget amount in any major category - Persom]e!; Boating Equipment and Associated Costs; Non-Expendable <br />Education Equipment; Supplies/Expendable Materials; Other, require the written approval of the Chief of the <br />Division of Watercraft. <br /> <br /> We have assigned # 16-92 to your grant application. Should you ever have any questions on your grant, <br />please refer to your grant number when inquiring. It is very important that you always use your grant number. <br />We get a very large quantity of applications, and it will be very helpful in processing your grant if the grant <br />number is consistently used. <br /> <br /> The Division of Watercraft will be holding an adr~strator's information meeting for all 1992 boating <br />safety education grant recipients on Thursday, April 30, 1992. A registration form and related information is <br />enclosed. Due to space restrictions, please limit the number of persons attending to no more tha]u two. Return <br />the registration form to: ODNR - Division of Watercraft, 1952 Belcher Drive, Columbus, Ohio 43224, Attention: <br />Sharon Luczaj. Please set this day aside so that you or your representative can attend. This administrator's <br />meeting is one of the mandatory training sessions for grant recipients. During this information session, we will <br />discuss all aspects of the grant requirements includin~ paperwork, audit track requirements and recc~nendations, <br />suggestions on boating safety education course implementation and content, etc. We are sure you will have many <br />questions, regal-ding the above, and we will try to answer any and all as thoroughly as possible.
