No. 7101-9S, submitted <br />originally on 12-18-95) 0N.1-16-96. <br /> <br />ORDINANCE NO. <br /> 2--!~6 <br /> <br />BY:Boscia, Corrigan, Flannery, <br /> George, Seelie, Smith <br /> <br /> A=N ORDINANCE authorizing and directing the Director of <br />Finance to enter into an agreement with Blue Cross (Super Blue <br />Plus and HMO), Kaiser (HMO) and Emerald Health Network (HMO) and <br />to pay from City fund~ the premium costs of hospitalization <br />coverage, in an amount not to exceed that appropriated by Council <br />in the 1996 Appropuiation Ordinance, for persons enrolled in the <br />City's hospitalization plans for Fiscal Year 1996. <br /> <br /> WHEREAS, th~i~ Codified Ordinances of the City of <br />Lakewood authorize the Director of Finance to pay from City funds <br />the actual premium costs of hospitalization coverage for persons <br />enrolled in the City's hospitalization plan, and <br /> <br /> WHEREAS, following a competitive selection process, <br />Blue Cross, Kaiser and Emerald Health Network have submitted the <br />best hospitalization packages considering cost, convenience of <br />employees and scope of coverage, and <br /> <br /> WHEREAS, it is in the best interest of the City to <br />enter into an agreeme]~h with Blue Cross (Super Blue Plus and <br />HMO), Kaiser (}tMO)~ and Emerald Health Network (HMO) for <br />hospitalization coverage; now, therefore, <br /> <br /> BE IT ORDAINED BY THE CITY OF LA/fEWOOD, STATE OF OHIO: <br /> <br /> Section 1. That the Director of Finance, on behalf of <br />the City, is hereby a~thorized and directed to enter into an <br />agreement with Blue Czoss (Super Blue Plus and HMO), Kaiser (HMO) <br />and Emerald Health Network (HMO) and pay from City funds the <br />actual premium costs of hospitalization coverage, for persons <br />enrolled in the City's hospitalization plan in accordance with <br />the Codified Ordinances of the City of Lakewood. <br /> <br /> Section 21. It is found and determined that all formal <br />actions of this Council concerning and relating to the passage of <br />this resolution were a<~opted in an open meeting of this Council, <br />and that all such deliberation of the Council and of any of its <br />committees that result~-~d in such formal action were in meetings <br />open to the public in compliance with all legal requirements. <br /> <br />Adopted <br /> <br />Approved: <br /> <br />President. <br /> <br />~le'~k ' <br /> <br /> <br />
