1.0 EXECUTIVE SUMMARY <br /> <br />This report documents a plan for the remediation of Assessment of the properties located at 11726-28 <br />Detroit Avenue and I385-87 Hird Avenue, l. akewood, CuyahOga County, Ohio (Property). The Property <br />consists of three buildings on a parcel consisting of approximately 18,500 square feet. The buildings <br />include one 2-story building with a partial basement (Building I), one 2-s~ory building with a crawl space <br />(Building 2), and one 1-story garage building with no basement (Building 3). This remediation will <br />support a voluntary action at the Property under the Ohio EPA Voluntary Action Program (VAP), with <br />the objective of obtaining a No Further Action (NFA) Letter from a VAP Certified Professional. <br /> <br />According to VAP Rules (OAC 3745~300), an '~ldentified Area" means any location at a prope~ at <br />which hazardous substances or petroleum are known or' suspected to be present. Based on the <br />information developed during a Phase I Property Assessment, Hull & Associates, Inc. (HA1) has <br />determined that the following Identified Areas are present at the Property. <br /> <br />IDENTIFIED AREAS OF CONTAMINATION <br />Based upon HAI's review of documents and the Property reconnaissance, the following Identified Areas <br />were identified: <br /> <br />Sanborn Fire Insurance maps dated 1912 indicate that one 500-gallon gasoline UST <br />system Previously existed at the north end of Building 3. The Prope~ is not listed as <br />reporting any UST releases, and there is no information regarding UST activity on the <br />Property at the City of Lakewood Hre Prevention Bureau. Potential chemicals of concern <br />(COCs) include lead, tOtal petroleum hydrocarbons and benzene, wluene, ethylb~nzene <br />and xylenes (BTBX)~ The UST eligibility investigation conducted by HA1 in June 200I, <br />determined that no COCs were detected'in the soil or groundwater above BUSTR action <br />levels or above VAP standards for reSidential land use. <br /> <br />Asbestos-containing materials were identified in the follOwing areas: <br /> <br />- pipe insulation, wall plaster and ceiling plaster in Building 2; and <br />- wall plaster, floor tile and mastic in Building 1. <br /> <br /> <br />
