3anuary 1 to March 31 <br /> April i to .Tune 30 <br />July I to September 30 <br />October I to December 31 <br /> <br />Employees who: are eligible for a bonus in any quarter may elect, at the emp oyees <br />discretion; to bank the 8 hour bonus in a Personal Leave Bank. Any time banked in <br />this manner must be utilized prior to the end of the following Calendar Quarter. If said <br />time is not utilized, it shall be paid to the employee at the completion of the following <br />quarter.. Time Placed in the Personal Leave Bank may be utilized by the employee in <br />increments of not less than one hour with advance authorization from the employee's <br />immediate supervisor. <br /> <br /> 149.12 CZTY'S RZGHTS RESERVED. <br /> Nothing herein contained shall be construed to create any vested right. The City <br />hereby expressly reserves all rights and powers to amend, alter, repeal or revoke any <br />provision of this chapter and any interest, privilege, benefit or right created thereby. <br /> <br />149.13 SALARY DEDUCTZONS FOR HOSPZTALIZATION INSURANCE. <br />The Director of Finance pursuant to Ohio R.C. 1739.15, is hereby authorized to <br />deduct from the salaries or wages of any municipal employee subscribing to any <br />nonprofit hospital service plan, incorporated and operating under the provisions of Oh!o_ <br />R.C. 1739.01 et seq. such amount monthly as may be necessary for such employee s <br />subscription payment to such plan and as shall have been stipulated by such employee <br />in a wri~en authorization filed with the Director of Finance requesting such deductions. <br />The Director of Finance is hereby authorized to make remittances of the aggregate <br />amount of sums so authorized to be deducted to the corporation operating such <br />non profit hospital service plan during the mOnth following the date of such deductions. <br /> <br /> 149.14 HOSPZTAL1'ZATION COVERAGE FOR EMPLOYEES. <br /> La' (a)... For th.e,.purp, ose.s..of t,.h.!s se.~io.n, "Ci~/.e~ployees" means employees of <br /> KeWO00 Municipa~ ~:ourc ano me cio/ot ~aKewooo, Duc excludes members of Council, <br /> part-time hourly employees, temporary employees, seasonal employees, school guards, <br /> public works inspectors, and members of the Civil Service Commission. Medical benefits <br /> shall become effective for each eligible employee on the first day of the calendar month <br /> following the mqnth~in which the individual is hired. <br /> <br /> (b) The Director of Finance is hereby authorized to pay from City funds on <br />behalf of each City employee wh° i,s. not covered by.a collective bargaining agreement <br />and who is a participant in the City s primary groop hospitalizaUon plan as adopted by <br />the City, the full monthly cost of said coverage for single or family coverage basis as <br />selected by the employee. <br /> <br /> (c) The Director of Finance is hereby authorized to pay from City funds on <br />behalf of each City employee who is not covered by a collective bargaining agreement <br />and who is a participant in an authorized Health MaintenanCe Organization (HMO) the <br />full monthly cost of said coverage for sing e or family coverage as selected b~ the <br />employee provided the full monthly cost does not exceed the amount paid b the Ci <br />for participants in the City's primary grOup hospitalization plan. Any additionalYcost sh~ <br />be borne by the employee and paid for through a payroll deduction. <br /> <br /> (d) The director of Finance is hereby authorized to continue to pay to employees <br />hired prior to April 3, 1978, a cash payment in lieu of medical benefi~ in the following <br />amounts: <br /> _1) .... Family .Plan up to $70.26 <br /> 2) <br /> Single Plan up to $42.31 <br /> <br />This section does not confer the right upon any employee to elect a cash payment in <br />lieu of medical .benefits regardless of the hire date of the employee, if the employee has <br />not formerly done so. <br /> <br /> (e) Such payment by the City shall be distributed among City funds according <br />to the enrollment of each division participatino in the City's emDlovee hn~nifRli?~i*inn <br />plan. Cost allocation shall be determined'base~J on the nu'mber o~f <br />division participaUng in the hospitalization plan. The. total costs of hospitalizaton <br />services shall be allocated according to the percent of e.n, rollment each division <br />represents as compared to the total enrollment on the City s hospitalizaUon plans. <br />Where there is no provision in grant programs to pay for emp oyee hospitalization costs, <br /> <br />18 <br /> <br /> <br />
