EXHIBIT <br />LEASE AGREEMENT <br /> <br />THIS LEASE, is entered into as of the day of , 2011, by and between, <br />Lakewood Senior Citizens, Inc. ("Landlord"), an Ohio Non-profit corporation, and the City of Lakewood <br />Police Department ("Tenant"). <br />WHEREAS, Landlord currently owns and operates a senior living complex commonly known as <br />the "Westerly Apartments", located at 14300 Detroit Avenue, Lakewood, Ohio; <br />WHEREAS, The City of Lakewood Police Department desires to lease a Studio Apartment (the <br />"Apartment") within the Senior Living Complex from Landlord for use as a community policing office <br />for the benefit of the citizens of the City of Lakewood; <br />WHEREAS, Landlord wishes to lease the Apartment to the City of Lakewood Police Departrnent <br />for such use as a community policing office. <br />NOW, THEREFORE, in consideration of the rent to be paid and the covenants to be performed <br />by Tenant, Landlord hereby leases to Tenant, and Tenant hereby leases from Landlord, the Apartment <br />pursuant to the terms and conditions set forth herein below: <br />1. PARTIES <br />Lakewood Senior Citizens, Inc. / dba Westerly Apartments <br />14300 Detroit Ave. <br />Lakewood, OH 44107 <br />City of Lakewood -Police Department <br />12650 Detroit Avenue <br />Lakewood, OH 44107 <br />2. DESCRIPTION OF APARTMENT -The Apartment which is leased hereunder is identified as <br />Unit No. 116, is located on the 15` floor, and consists of approximately 380 square feet. <br />3. TERM -This Agreement shall be month-to-month, beginning January 1, 2011, until terminated <br />by either party. <br />4. RENTAL FEE -Tenant will pay the sum of $423 per month, as rental for the Apartment. One <br />month's rent of $423 will be paid in advance. Rent is due by the l Os' day of each month. <br />5. CONDITIONS -CITY OF LAKEWOOD -Tenant agrees to comply with and carry out the <br />following condition: <br />
