der some other medical plan outside of the City. The cash payment in <br />lieu of medical benefits for the year 2005 shall be seven hundred twen- <br />ty dollars ($720.00). The Director of Finance shall annually review the <br />cost of providing medical benefits to each employee and adjust the cash <br />payment in lieu of medical benefits accordingly. <br />In the event that an employee receives a cash payment in lieu of medi- <br />cal benfits and in the same calendar year, due to a change in status, <br />elects to enroll in any of the City's major medical plans, the employee <br />will be required to reimburse the City a prorated amount based on the <br />number of months of enrollment in the plan for that calendar year, <br />(f) Such payment by the City shall be distributed among City funds ac- <br />cording to the enrollment of each division participating in the City's <br />employee hospitalization plan. Cost allocation shall be determined <br />based on the number of employees in each division participating in the <br />hospitalization plan. The total costs of hospitalization services shall be <br />allocated according to the percent of enrollment each division repre- <br />sents as compared to the total enrollment on the City's hospitalization <br />plans. Where there is no provision in grant programs to pay for em- <br />ployee hospitalization costs, the Director of Finance will allocate that <br />cost to the funds based upon the total allocation of all other hospitaliza- <br />tion costs. <br />(g) The Director of Finance is hereby authorized to pay from City funds <br />one-half of the actual premium rate of dental insurance coverage as <br />may be established by the City for all annual salaried employees in- <br />cluding Foremen Class A employees and the Chiefs of the Divisions of <br />Police and Fire but excluding all other members of the Divisions of Po- <br />lice and Fire and full- time permanent hourly employees covered by a <br />collective bargaining agreement with the City.. <br />shall be and hereby is amended to read as follows: <br />149.14 HOSPITALIZATION COVERAGE FOR EMPLOYEES. <br />(a) For the purposes of this section, "eligible City employees" means em- <br />ployees of Lakewood Municipal Court and the City of Lakewood who <br />meet the eligibility criteria established by the City's heath and welfare <br />lp ans, but excludes members of Council, part-time hourly employees, <br />temporary employees, seasonal employees, school guards, °~~"-~li~rl~s <br />inspec-te~~ and members of the Civil Service Commission. Medical <br />benefits shall become effective for each eligible employee on the first <br />day of the calendar month following the month in which the individual <br />is hired. <br />(b) The Director of Finance is hereby authorized to pay from City funds on <br />behalf of each eli ible City employee who is <br />t;°° "°-°°~°~~° °°~°°m°°' °°a ~~ "° ~° a participant in the City's pri- <br />mary group hospitalization plan or dental plan as adopted by the City, <br />the full or a portion of the monthly cost of said coverage, including <br />claims costs and all administrative charges, for single or family cover- <br />age basis as selected by the employee and as established by Cit~policv <br />or an applicable collective bargaining_agreement. <br />
