As used in [he Regional Dwelling House Code, as adopted herein, certain terms <br />are defined as follows: <br />(a) "Building Official" means the Building Commissioner of the City. <br />(b) "Board of Building Code Appeals" means the Board of Zoning Appeals of <br />the City. <br />(c) "Board of Examiners" means the Board of Building Standards and Building <br />Appeals of the City. <br />(d) "Municipality" means the City of Lakewood. <br />(e) "City of Lakewood". The words "City of Lakewood" are hereby inserted Irr <br />any and all blank spaces provided in the Code for the name of the <br />municipality adopting the Code. Words and ternrs used in the Code shall be <br />construed to mean the words, terms and titles applicable to the City of <br />Lakewood. <br />1301.04 AMENDMENTS, ENACTMENTS OR REPEALS. <br />The following chapters, sections or subsections of the Regional Dwelling House <br />Code are hereby amended, enacted or repealed as follows: <br />RDH 1503.10, Chapter 1, Nonstructural Alterations (Amended). <br />When the cost of alterations or repairs within any two (2) year period exceeds <br />seventy-five percent (75%) of the value of the existing building or other <br />structure, such building or other structure shall be made to comply with the <br />requirements for new buildings and other new structures. <br />RDH 1505.02, Application (Amended). <br />An application for a permit shall be filed with the Building Official on a form <br />prescribed by the Building Official by the person, corporation, partnership, firm <br />or other business association who shall perform the work for which the permit <br />was sought. <br />RDH 1505.05, Issuance of Permit (AmendedZ to enact subsection (c) as <br />follows: <br />(c) No permit shall be issued until the applicant has registered on a form <br />prescribed and furnished by the Administrator of the City Income Tax, the name <br />of the general contractor, subcontractors and all other persons involved with the <br />actual construction, the place of business of each, and the person or persons to <br />whom correspondence shall be directed for each such contractor, subcontractor <br />or other person involved. Upon issuance of the permit, the Building <br />Commissioner shall transmit such form to the City Income Tax Administrator. <br />RDH 1505.08, Time Limitation (Amended <br />Any permit issued shall become invalid unless the work authorized by it shall <br />have been commenced within tluee (3) months after its issuance. Any permit <br />issued shall become invalid if the work authorized by it is not completed within <br />six (6) months after its issuance. The Building Official, for good cause as <br />evidenced by a written request by the permit holder, may grant up to four (4) <br />extensions of time for either of the above not exceeding ninety (90) days each. <br />RDH 1509.03, General Penalty (AmendedZ <br />
