TH~S AGREE~IT, is entered into by and between the Western Reserve Area Agency <br /> <br />on Aging, being ~he duly authorized agen~ of ~he Board of County Commissioners of <br />Cuyahoga County and serving ~he counties of Cuyahoga, Geauga, Lake, Lorain and <br />Medina, Ohio (Agency), and <br /> <br /> as ~he implementing authority (Grantee) for ~he purpose of providing Title III <br /> service(s) as described in ~he Older Americans Act of 1965, as amended, to persons <br /> sixty (60) years of age and older. <br /> <br /> The Agency and Grantee hereby agree as follows: <br /> <br /> 1. Scope of Service(s]. The Grantee agrees to provide the Title III <br />service(s) contained on the Contract Services page(s) attached for a twelve month <br />period commencing January 1, 1990 through and including December 31, 199~. The <br />Grantee agrees to provide such service(s) according to the procedures described in <br />the Grantee's proposal .for 1990 Title III funds,.as amended and approved by the <br />Agency. The Grantee assures that during this agreement period it has the <br />capability to and agrees to provide such service{s) as referred to above and in <br />accordance with the Ohio Department of Aging [ODA) taxonomy of services and Agency <br />clarifications to said taxonomy. <br /> <br /> 2. Grant. The Agency agrees to reimburse the Grantee with Title III program <br />funds for the cost of the service(s) detailed in the attached Contract Services <br />page(s) up ~o the amoun~(s) in section B, line 1 on the said page(s).
